目的探讨游离足趾甲床瓣在手指甲床不同类型缺损中的临床应用.方法对12例手指甲床缺损的患者,根据甲床缺损形状、类型采用不同形状游离甲床瓣移植修复,5例足趾供区创面直接缝合皮肤闭合；7例足趾供区以足趾腓侧趾背动脉为蒂转移皮瓣+植皮术修复.结果 12例足趾甲床瓣均获得存活,足趾供区转移皮瓣及植皮全部存活.术后随访1～ 18个月,平均7个月,手指甲外观满意,其中4例指甲出现不同程度的挛缩,手指功能恢复良好,供区创面未见明显瘢痕及水泡形成,足趾行走功能无明显影响.结论游离足趾甲床瓣在手指甲床不同类型缺损的应用是一种理想的方法.

Objective To investigate the clinical application of free nail bed flap from the toe in the treatment of different types of nail bed defects of the finger.Methods Twelve cases of fingernail bed defects were treated with transplantation of free nail bed flap of various shapes from the toe based on the type and shape of nail bed defect.Donor site of the toe could be closed directly in 5 cases,while a fibular toe flap pedicled on the dorsal digital artery and skin graft was used to close the donor site wound in 7 cases.Results The 12 transferred toe nail bed flaps all survived,as well as the fibular toe llaps and skin grafts at the donor site.Patients were follow-up for 1 to 18 months,with an average of 7 months.Appearance of the reconstructed fingernails was satisfactory.Varying degrees of contracture of the nail were seen in 4 cases.Finger function was good.There was no obvious scar and blister formation at the donor site.Walking was not affected.Concclusion Transfer of free nail bed flap from the toe is an ideal method to t-eat different types of nail bed defects of the finger.
