目的观察重型乙型肝炎前期(以下简称重肝前期)细胞免疫及体液免疫状况.方法选取重肝前期患者56例,慢性乙型肝炎患者40例,健康志愿者20例,采用流式细胞仪检测外周血淋巴细胞CD3+、CD4+、CD8+、CD3-/CD19+(B细胞)等亚群表达百分比,免疫散射比浊法检测血清IgG、补体C3,并进行统计学分析.结果与慢性乙肝组及健康对照组相比,重肝前期组CD8+T细胞百分率明显降低(P＜0.01或P＜0.05),但其CD4+百分率及CD4+/CD8+的比值显著高于慢性乙肝组(P＜0.01或P＜0.05)；慢性乙肝组与重肝前期组外周血B细胞的平均百分率及血清IgG水平差异无统计学意义(P＞0.05)；重肝前期组血清补体C3水平显著下降(P＜0.01或P＜0.05).结论重型乙肝前期患者存在一定程度的免疫功能紊乱,相对慢性乙肝而言,细胞免疫相对亢进,体液免疫相对受抑.
목적 관찰중형을형간염전기(이하간칭중간전기)세포면역급체액면역상황.방법 선취중간전기환자56례,만성을형간염환자40례,건강지원자20례,채용류식세포의검측외주혈림파세포CD3+、CD4+、CD8+、CD3-/CD19+(B세포)등아군표체백분비,면역산사비탁법검측혈청IgG、보체C3,병진행통계학분석.결과 여만성을간조급건강대조조상비,중간전기조CD8+T세포백분솔명현강저(P＜0.01혹P＜0.05),단기CD4+백분솔급CD4+/CD8+적비치현저고우만성을간조(P＜0.01혹P＜0.05)；만성을간조여중간전기조외주혈B세포적평균백분솔급혈청IgG수평차이무통계학의의(P＞0.05)；중간전기조혈청보체C3수평현저하강(P＜0.01혹P＜0.05).결론 중형을간전기환자존재일정정도적면역공능문란,상대만성을간이언,세포면역상대항진,체액면역상대수억.
Objective To study cellular and humoral immune status on prophase of severe hepatitis B(PSHB).Methods 56 cases of PSHB patients,40 cases of chronic hepatitis B (CHB) patients and 20 cases of healthy volunteers were enrolled for detection of CD3 +,CD4 +,CD8 + and CD3-/CD19 + (B cells) lymphocyte subsets in peripheral blood by flow cytometry.Serum IgG and complement C3 was detected by immunoturbidimetry and analyzed statistically.Results Compared with CHB group and healthy control group,percentage of lymphocyte subsets CD8 + were significantly lower in PSHB group(P ＜ 0.01 or P ＜ 0.05).while the percentage of lymphocyte subsets CD4 + and ratio of CD4 +/CD8 + in PSHB group was obviously higher than those in CHB group (P ＜ 0.01 or P ＜ 0.05).In addition,There was no significant difference on the percentage of B cell and level of serum IgG between PSHB group and CHB group (P ＞ 0.05),while the level of serum complement C3 in PSHB group were significantly lower than those in CHB group and healthy control group (P ＜ 0.01 或 P ＜ 0.05).Conclusion PSHB has a certain degree of cellular immune dysfunction,which characterized by cellular immune function hyperfunction and humoral immune suppression.
