目的 检测转移性肾癌与非转移性肾癌细胞因子受体4(CXCR4)表达强度的差异,以及肾癌常见转移部位淋巴结与肾癌罕见转移部位输尿管基质细胞衍生因子-1(SDF-1)表达强度的差异,探讨SDF-1及其受体生物学轴(SDF-1/CXCR4生物学轴)与肾癌转移的关系.方法 选取转移性肾透明细胞癌标本32例,非转移性肾透明细胞癌标本31例.采用免疫组织化学方法检测转移性肾癌与非转移性肾癌CXCR4表达强度的差异.选取肾透明细胞癌患者淋巴结标本30例;肾透明细胞癌患者输尿管标本30例.采用免疫组织化学的方法检测肾癌常见转移部位淋巴结与肾癌罕见转移部位输尿管SDF-1表达强度的差异.结果 32例转移性肾透明细胞癌CXCR4表达强度评分为(126.72±74.72)分,31例非转移性肾透明细胞癌CXCR4表达强度评分为(63.39±60.69)分.两者差异有统计学意义(P<0.05).30例淋巴结SDF-1表达强度评分为(145.00±46.42)分,30例输尿管SDF-1表达强度评分为(63.50±23.49)分.两者差异有统计学意义,P<0.05.结论 转移性肾癌比非转移性肾癌CXCR4表达强度高.CXCR4表达强度高的肾癌更容易发生转移.肾癌常见转移部位淋巴结比肾癌罕见转移部位输尿管SDF-1表达强度高.SDF-1表达强度高的部位更容易发生转移.

Objective To investigate the relationship between stromal cell derived factor-1 (SDF-1)/CXC chemokine receptor 4 (CXCR4) biological axis and renal cell carcinoma metastasis,by detecting differential expression of CXCR4 between metastatic renal cell carcinoma and non-metastatic renal cell carcinoma and the difference in SDF-1 expression between the lymphnode and ureter.Methods Metastatic renal clear cell carcinoma specimens were collected from 32 cases,and non-metastatic renal clear cell carcinoma specimens from 31 cases.The difference in CXCR4 expression between metastatic renal cell carcinoma and non-metastatic renal cell carcinoma expression was detected by using immunohistochemistry.The lymph node specimens (n =30) and ureteral specimens (n =30) were selected from patients with renal cell carcinoma.The difference in the SDF-1 expression between the lymph node and ureter was compared.Results In 32 patients with metastatic renal cell carcinoma,the CXCR4 expression score was (126.72 ± 74.72),and that in 31 cases of non-metastatic renal cell carcinoma was (63.39 ± 60.69) (t test,P <0.05).In 30 cases of lymph node specimens,the SDF-1 expression score was 145.00 ±46.42,and that in 30 cases of ureteral specimens was 63.50 ± 23.49,P < 0.05.Conclusion CXCR4 expression in metastatic renal cell carcinoma was higher than in non-metastatic renal cell carcinoma.The SDF-1 expression in lymph nodes was higher than in the ureter.
