目的 检测肥胖大鼠袖状胃切除术后循环ghrelin水平及下丘脑雷帕霉素靶蛋白(mTOR)通路活化状态.方法 将15只肥胖SD大鼠随机分为3组:假手术组、袖状胃切除术组和保留部分胃底的袖状胃切除术组.术后2周记录每日摄食量,酶联免疫吸附试验(ELISA)检测血清ghrelin水平,Western blot法检测磷酸化mTOR(pmTOR)和磷酸化P70 S6K(pP70 S6K)表达水平.结果 袖状胃切除术显著降低摄食量[(20.10±1.95)g比(10.44±1.18)g,P<0.05]和血清ghrelin水平[(1556.20±143.35) μg/L比(1162.20±170.96) μg/L,P<0.05],pmTOR和pP70 S6K水平较对照组分别升高2.54倍和3.60倍.保留部分胃底的袖状胃切除术显著降低摄食量,但血清ghrelin 水平无显著变化,pmTOR和pP70 S6K水平较对照组轻度降低.结论 袖状胃切除术后循环ghrelin 水平显著降低,mTOR通路在负能量平衡下被异常激活.

Objective To evaluate the status of serum ghrelin level and mammalian target of rapamycin (mTOR) pathway in rat hypothalamus following sleeve gastrectomy.Methods Fifteen SD rats were divided into 3 groups:sham operation,sleeve gastrectomy and sleeve gastrectomy with partial fundus preservation.On the 2nd week post-operation,the volume of food intake was recorded.Serum level of ghrelin was determined by using ELISA and mTOR pathway status was evaluated by Western blotting.Results Sleeve gastrectomy significantly reduced food intake [(20.10 ± 1.95) g vs.(10.44 ± 1.18) g,P <0.05]and serum ghrelin level [(1556.20 ± 143.35) μg/L vs.(1162.20 ± 170.96) μg/L,P <0.05].pmTOR and pP70 S6K level were up-regulated by 2.54 and 3.60 times in sleeve gastrectomy group,respectively.Sleeve gastrectomy with partial fundus preservation reduced food intake,but not ghrelin level,and pmTOR and pP70 S6K levels were slightly down-regulated.Conclusion Serum level of ghrelin was decreased while mTOR pathway was activated under negative energy balance following sleeve gastectomy.
