目的 在杂种犬体内建立带有原位椎旁肌-肌骨瓣的新型腰椎后路植骨方法,评价植骨融合效果,并与横突间融合进行对比.方法 将32条杂种犬随机分为实验组和对照组.8周和16周处死犬.取材后行放射线、组织学及血清骨生化指标检测.结果 术后8周及16周的放射线及组织学评分结果均显示实验组明显高于对照组,差异有统计学意义(P<0.05);术后8周实验组和对照组的放射线融合率分别为57.1%、21.4%;16周分别为93.8%、62.5%,实验组的融合率均高于对照组;血清骨生化指标中骨碱性磷酸酶(BALP)、骨钙素(OC)、Ⅰ型胶原末端肽(CTX-1)3项实验组均明显高于对照组,实验组始终处于较高的骨转换状态.结论 该植骨方法植骨位置确切、牢靠、融合率高,能够有效减少假关节的形成.

Objective To construct a novel posterior lumbar spine fusion canine model,evaluate the result of bone graft healing and compare with the conventional posterolateral lumbar intertransverse process fusion.Methods Thirty-two mongrel dogs were randomly assigned to experimental group or control group.The animals were sacrificed at 8th week or 16th week after surgery.The lumbar spines were evaluated radiologically and histologically,and the serum biochemical indicators of bone were examined.Results The radiographic and histological analysis at 8 and At 16th week after the surgery demonstrated that the score of fusion in the treatment group was statistically significantly higher than in the control group (P <0.05).At 8th week,the radiographic fusion rate was 57.1% and 21.4% in the treatment and control groups,respectively; and at 16th week,the fusion rate was 93.8% and 62.5%,respectively.The levels of bone alkaline phosphatase(BALP),osteocalcin(OC) and type Ⅰ collagen telopeptide(CTX-1) in the treatment group were significantly higher than in the control group.The dogs in the treatment group all the time were at higher state of bone turnover.Conclusion The posterior lumbar fusion is a novel and useful method that makes the bone graft position more precise and stable and increases the rate of fusion.This method can decreases the incidence of pseudarthrosis.
