目的 观察右美托咪啶对麻醉诱导和气管插管时血液动力学的影响.方法 选取30例心血管疾病的患者分为两组:研究组患者给予右美托咪啶和芬太尼(效应部位浓度为2 μg/L);对照组患者给予安慰剂和芬太尼(效应部位浓度为2 μg/L).应用靶控灌注系统给予异丙酚和芬太尼实施麻醉.记录患者心脏的收缩压(SBP)、舒张压(DBP)、心率(HR),并计算诱导期和插管期的参数变化.结果 诱导麻醉后,研究组患者的SBP[127 mmHg(1 mmHg =0.133 kPa)]较对照组患者的90 mmHg显著升高(P<0.05).实施插管后,对照组患者的SBP达到了160 mmHg,而研究组患者的SBP没有明显变化.由气管插管造成的SBP显著增加占研究组病例的3%,远低于对照组(70%,P<0.05).结论 右美托咪啶和芬太尼联用能抑制实施麻醉造成的血压降低以及减轻气管插管引起的心血管反应.

Objective To evaluate the effect of dexmedetomidine combined with fentanyl on hemodynamics.Methods Patients were assigned to two groups,research group (dexmedetomidine,effect-site concentration (ESC) of fentanyl is 2 μg/L) ; control group (placebo,ESC of fentanyl is 2 μg/L).Dexmedetomidine or placebo saline was administered 15 minutes before anesthetic induction.Anesthesia was induced with propofol and fentanyl using a target-controlled infusion system.Hemodynamic parameters:systolic (SBP) and diastolic blood pressures (DBP),and heart rate (HR) during anesthetic induction were measured and the percent changes were calculated for both induction and intubation.Results After inducing anesthesia,SBP was significantly higher in research group [127 mmHg(1 mmHg=0.133 kPa)] than control group (90 mmHg).The SBP in control group reached 160 mmHg after intubation,but no significantchange in SBP was noted in research group.The percent increase in SBP due to tracheal intubation in research group was 3% and was significantly lower than that of control group (70%).Conclusion Dexmedetomidine combined with fentanyl during anesthetic induction suppresses the decrease in blood pressure due to anesthetic induction and also blunts the cardiovascular response to tracheal intubation.
