目的 在观察食管反流形成咽喷嘴基础上,探讨胃食管反流对气道的侵袭.方法 10只SD大鼠分A、B两组,5只兔为C组.动物麻醉后经胃向食管插入9F导管,A组切开甲状及环状软骨,经导管注射5%葡萄糖氯化钠;B和C组分别注射泛影葡胺及硫酸钡,透视观察咽部征象.结果 A组大鼠,随注射压力上升,咽部出现喷洒;B和C组动物,当压力分别达(150.0±9.8)、(187.8±8.3) mmHg(1 mmHg =0.133 kPa),观察到咽喷嘴;随后造影剂进入喉及气管,动物剧烈呛咳.结论 通过构建胃食管高位反流模型证实咽喷嘴和喷洒是胃食管反流对气道侵袭的基础,为探讨胃食管反流致呼吸道疾病的发病机制提供了新思路.

Objective To explore the insult of gastroesophageal reflux (GER) on airway.Methods Ten Sprague-Dawley (SD) rats were randomly divided into group A and group B.Five New Zealand white rabbits were assigned to group C.A 9F catheter was inserted into the esophagus through animal's stomach under anesthesia.The thyroid and cricoid cartilages in group A were incised longitudinally.Five percent glucose and sodium chloride were injected into esophagus through a catheter in group A.For the first time,this study creates a high-located GER animal model and illustrates a morphology of pharyngeal nozzle and its spray phenomenon,which are the bases of GER in sult on airway and provide a new thinking for exploring the pathophysiological medianism of respiratory diseases caused by GER.Meglumine diatrizoate and 40% barium sulfate were respectively injected into esophagus in groups B and C with pressure.The manifestation of pharynx and upper airway was observed under fluoroscopy and recorded by video.Results As injecting pressure increased in group A,spray with various parabola appeared from the distal pharynx.Pharyngeal nozzles were seen in groups B and C while the refluxing pressure increased to (150.0 ±9.8) mmHg (1 mmHg =0.133 kPa) and (187.8 ± 8.3) mmHg respectively.The animals gasped severely when the contrast agents entered instantly into the upper airway.Conclusion A high-located GER animal model is firstly created and it was confirmed that the pharyngeal nozzle and spray are the bases of GER insult on airway,which may provide a new thinking for exploring the pathophysiological machanism of respiratory diseases caused by GER.
