目的 观察超级化激活环核苷酸调控通道基因家族-2(HCN2)在风湿性心脏病二尖瓣狭窄伴心房颤动患者右心房组织中的表达,探讨伴发心房颤动的发生机制.方法 收集43例风湿性心脏病二尖瓣狭窄患者右心耳组织,实验组为并发心房颤动患者(AF组),对照组为窦性心律患者(SR组),采用荧光定量聚合酶链反应法(FQ-PCR)、Western blot法和免疫组织化学法,检测风湿性心脏病二尖瓣狭窄患者右心耳组织中HCN2 mRNA及蛋白的表达.结果 FQ-PCR结果显示AF组和SR组HCN2 mRNA表达量分别为0.70±0.14和0.39±0.08;而Western blot结果显示AF组和SR组HCN2蛋白表达量分别为0.52±0.19和0.21 ±0.12,AF组均明显高于SR组(P<0.01);免疫组织化学结果显示AF组右心耳心肌细胞溶解、肥厚并广泛间质纤维化,且HCN2蛋白表达量明显高于SR组(P<0.01).结论 HCN2在风湿性心脏病二尖瓣狭窄伴心房颤动患者右心耳组织中表达明显上调,并参与其心房颤动的发生发展过程.

목적 관찰초급화격활배핵감산조공통도기인가족-2(HCN2)재풍습성심장병이첨판협착반심방전동환자우심방조직중적표체,탐토반발심방전동적발생궤제.방법 수집43례풍습성심장병이첨판협착환자우심이조직,실험조위병발심방전동환자(AF조),대조조위두성심률환자(SR조),채용형광정량취합매련반응법(FQ-PCR)、Western blot법화면역조직화학법,검측풍습성심장병이첨판협착환자우심이조직중HCN2 mRNA급단백적표체.결과 FQ-PCR결과현시AF조화SR조HCN2 mRNA표체량분별위0.70±0.14화0.39±0.08;이Western blot결과현시AF조화SR조HCN2단백표체량분별위0.52±0.19화0.21 ±0.12,AF조균명현고우SR조(P<0.01);면역조직화학결과현시AF조우심이심기세포용해、비후병엄범간질섬유화,차HCN2단백표체량명현고우SR조(P<0.01).결론 HCN2재풍습성심장병이첨판협착반심방전동환자우심이조직중표체명현상조,병삼여기심방전동적발생발전과정.
Objective To research the expressions of hyperpolarization-activated cyclic nucleotide-gated cannel family 2 (HCN2) in atrial fibrillation patients with mitral stenosis of rheumatic heart disease (RHD),and to explore the possible adjustment mechanism of atrial fibrillation in RHD.Methods A toral of 43 patients with RHD were divided into two groups:the atrial fibrillation group (AF) and the sinus rhythm group (SR); Real-time fluores-cence quantitative polymerase chain reaction (FQ-PCR),Western blotting and the Immunohistochemistry methods were used to analyze the expressions of HCN2 in a toral of 43 patients with RHD.Results In the FQ-PCR,the levels of expressions of HCN2 gene in AF group and SR group were 0.70 ± 0.14 and 0.39 ± 0.08 ; then in the Western blot,the levels of expressions of HCN2 in AF group and SR group were 0.52 ±0.19 and 0.21 ±0.12,the levels of expressions of HCN2 channal in AF group were higher than the SR group (P <0.01) ; In the immunohistochemistry,myocardial hypertrophy,severe myolysis and extensive fibrosis in interstitial space of the atria were found in AF group,and the the expressions of HCN2 protein in atrial fibrillation patients with mitral stenosis of RHD were also higher than the sinus rhythm group (P <0.01).Conclusion The HCN2 gene and protein were highly expressed in atrial fibrillation in RHD,and its expression level were play an important role in atrial fibrillation in RHD.
