目的 观察十二指肠空肠旁路(DJB)手术对2型糖尿病SD大鼠骨骼肌和心肌组织中葡萄糖转运体4(GLUT-4)之间的影响.方法 取40只8周大SD大鼠,将其分为糖尿病手术组(To组)、糖尿病对照组(Tc组)、正常手术组(No组)和正常对照组(Nc组),每组10只,检测各组大鼠术前后的体质量、空腹血糖、胰岛素、胰岛组织病理形态,GLUT-4蛋白表达水平.结果(1)To组体质量从(293.20±29.16)g下降到(251.75 ±35.49)g,差异无统计学意义(P>0.05).(2)To组血糖值从术前的(22.71 ±5.26) mmol/L下降到术后8周的(4.05±2.39) mmol/L(P <0.05),且接近No组术前术后的血糖值.(3)胰岛素值从术前的(11.61 ±0.51) mU/L下降到(6.35±3.38) mU/L(P <0.05),且与术前No组的胰岛素值相近.(4)Tc组胰岛有着明显的病理改变,To组接近No、Nc组,有逐渐恢复正常的趋向.(5)To组术后8周骨骼肌和心肌里的GLUT4蛋白表达水平明显升高(P<0.05).结论 DJB手术并不能减轻2型糖尿病SD大鼠的体质量,但可以降低大鼠血中的血糖和改善胰岛素抵抗,并且能提高2型糖尿病SD大鼠骨骼肌和心肌中GLUT4蛋白的表达水平.

목적 관찰십이지장공장방로(DJB)수술대2형당뇨병SD대서골격기화심기조직중포도당전운체4(GLUT-4)지간적영향.방법 취40지8주대SD대서,장기분위당뇨병수술조(To조)、당뇨병대조조(Tc조)、정상수술조(No조)화정상대조조(Nc조),매조10지,검측각조대서술전후적체질량、공복혈당、이도소、이도조직병리형태,GLUT-4단백표체수평.결과(1)To조체질량종(293.20±29.16)g하강도(251.75 ±35.49)g,차이무통계학의의(P>0.05).(2)To조혈당치종술전적(22.71 ±5.26) mmol/L하강도술후8주적(4.05±2.39) mmol/L(P <0.05),차접근No조술전술후적혈당치.(3)이도소치종술전적(11.61 ±0.51) mU/L하강도(6.35±3.38) mU/L(P <0.05),차여술전No조적이도소치상근.(4)Tc조이도유착명현적병리개변,To조접근No、Nc조,유축점회복정상적추향.(5)To조술후8주골격기화심기리적GLUT4단백표체수평명현승고(P<0.05).결론 DJB수술병불능감경2형당뇨병SD대서적체질량,단가이강저대서혈중적혈당화개선이도소저항,병차능제고2형당뇨병SD대서골격기화심기중GLUT4단백적표체수평.
Objective To discuss the relationship between duodenal-jejunal bypass (DJB) sur-gery and glucose transporter-4 (GLUT-4) in skeletal musclc and myocardia of type 2 diabetic mcllitus (T2DM) SD rats.Methods Forty 8-week SD rats were randomly divided into four groups:T2DM control group (Tc group),T2DM-operation group (To group),normal-operation group (No group) and normal control group (Nc group).The weight,fasting blood glucose,insulin and the expression of GLUT-4 protein were measured and recorded respectively before and at 1 st,2nd,4th as well as 8 week after operation in all rats.Pathological changes in islet tissue were observed under microscope after HE staining.Results (1) The weight loss in To group showed no significant difference (P>0.05).(2) At 8th week after surgery the fasting blood glucose in To group was decreased to (4.05 ±2.39) mmol/L from (22.71 ±5.26)mmol/L pre-operation (P < 0.05).(3) The fasting blood insulin in To group was decreased to (6.35 ±3.38) mU/L from (11.61 ± 0.51) mU/L (P < 0.05),close to the preoperative value of insulin ; (4)The obvious pathological changes of islets were observed in Tc group,but those in To group were similar to No and Nc groups,and had a tendency to be gradually returned to the normal.(5) The expression of GLUT4 protein in skeletal muscle and myocardia was significantly increased.Conclusion Though DJB surgery can't reduce the weight of T2DM SD rats,this procedure can lower blood glucose level,improve insulin resistance in blood,and up-regulate the expression of GLUT4 protein in skeletal muscle and myocardia of T2DM SD rats.
