目的 通过植入不同长度的腔内袖管装置(ELS),观察十二指肠肠段旷置对肥胖大鼠能量代谢、血糖调控及胰腺胰高血糖素样肽-1受体(GLP-1R)表达的影响.方法 将雄性SD大鼠随机分为4组:假手术组(SO,n=8)、1 cm ELS管植入组(ELS-1,n=10)、4 cm ELS管植入组(ELS-4,n=8)、10 cm ELS管植入组(ELS-1O,n=9).比较置入ELS组与假手术组的糖尿病大鼠的体质量、能量消耗、胰岛素抵抗、GLP-1水平等多项参数.术后第8周,逆转录-聚合酶链反应(RT-PCR)检测胰腺GLP-1R mRNA的表达变化.比较不同程度的十二指肠旷置对于代谢结果及血糖调控的影响.结果 置入10 cm ELS的大鼠与SO组比较,体质量下降14%(P<0.05),总能量消耗高13% (P <0.05),静息能量消耗高9%(P<0.05),胰岛素抵抗明显改善(P<0.05).空腹GLP-1释放升高(P<0.05),胰腺GLP-1受体表达水平升高(P<0.05).而置入4 cm或1 cm ELS的大鼠与SO组比较,在能量消耗上差异无统计学意义(P>0.05),对血糖稳态的影响轻微(P<0.05).结论 十二指肠全段旷置能增加大鼠总能量消耗,尤其是静息能量消耗,改善血糖调控能力,提高胰腺GLP-1受体表达,部分旷置只能引起适当的体质量下降.

Objective By implantating endoluminal sleeve devices at different length,We sought to investigate the influence on energy metabolism,glucose regulation and the expression of glucagon-like peptide-1 receptor (GLP-1R) in the pancreas of diet-induced obese rats.Methods Sprague-Dawley rats were randomly divided into four groups:sham-operated animnals (SO,n =8),rats implanted with a 1 cm endoluminal sleeve devices (ELS) device (ELS-1,n =10),4 cm ELS (ELS-4,n=8),or 10 cm ELS (ELS-10; n =9).We examined the effects of ELS implanted in rats on body weight,energy expenditure,insulin resistance,levels of GLP-1 and so on,comparing them with sham-operated (SO) rats.After 8 weeks,reverse transcriptase-polymerase chain reaction (RT-PCR) was used to analysis the mRNA levels of GLP-1 receptor in the pancreas.To determine the extent of duodenal exclusion required to influence metabolic outcomes and glucose control,we compared the effects of implanting 10,4,or 1 cm ELS with SO.Results ELS-10 rats exhibited 13% higher total and 9% higher resting energy expenditure than SO con-trols (P < 0.05).ELS-10 rats also exhibited enhanced fasting GLP-1 secretion and improved GLP-1 receptor expression (P < 0.05),insulin resistance (P < 0.05) with a significantly 14% lower weight loss (P <0.05).While implantation of 4 or 1 cm ELS devices had no effect on energy expenditure and limited effects on glucose homeostasis (P > 0.05).Conclusion Here we report that fully excluding the duode-num with an ELS increased total and resting energy expenditure,improved glucose regulation,as well as the GLP-1 receptor in pancreatic.Exclusion of only a portion of the duodenum resulted in more modest weight loss.
