目的观察几丁质酶-3样蛋白-1(CHI3L1)在胆囊癌组织中的表达,探讨CHI3L1蛋白与胆囊癌侵袭、转移及预后的关系.方法采用免疫组织化学链霉菌抗生物素蛋白-过氧化物酶(SP)法检测65例胆囊癌患者CHI3L1蛋白表达,比较CHI3L1蛋白表达与胆囊癌患者性别、年龄、肿瘤大小、有无胆囊结石、生长方式、淋巴结转移、远处转移、血管、淋巴管、神经侵犯及TNM分期之间的关系.结果胆囊癌患者中CHI3L1蛋白表达水平明显高于对照组(4.45±1.20比0.55±0.94,P＜0.05).CHI3L1蛋白表达与胆囊癌组织的生长方式、T分期、淋巴结转移、远处转移及TNM分期均明显相关(P＜0.05).生存分析表明CHI3L1阳性患者术后生存期明显短于CHI3L1阴性患者,差异有统计学意义[(13.0±-7.6)个月比(30.0±6.4)个月,P＜0.05].结论 CHI3L1表达与胆囊癌侵袭、转移及预后相关.
목적 관찰궤정질매-3양단백-1(CHI3L1)재담낭암조직중적표체,탐토CHI3L1단백여담낭암침습、전이급예후적관계.방법 채용면역조직화학련매균항생물소단백-과양화물매(SP)법검측65례담낭암환자CHI3L1단백표체,비교CHI3L1단백표체여담낭암환자성별、년령、종류대소、유무담낭결석、생장방식、림파결전이、원처전이、혈관、림파관、신경침범급TNM분기지간적관계.결과 담낭암환자중CHI3L1단백표체수평명현고우대조조(4.45±1.20비0.55±0.94,P＜0.05).CHI3L1단백표체여담낭암조직적생장방식、T분기、림파결전이、원처전이급TNM분기균명현상관(P＜0.05).생존분석표명CHI3L1양성환자술후생존기명현단우CHI3L1음성환자,차이유통계학의의[(13.0±-7.6)개월비(30.0±6.4)개월,P＜0.05].결론 CHI3L1표체여담낭암침습、전이급예후상관.
Objective To investigate the value of chitinase-3-like protein 1 (CHI3L1) expression of tissue samples in estimating the prognosis of gallbladder cancer.Methods The CHI3L1 protein expression in tumor tissue was measured by immunohistochemistry in 65 patients with gallbladder cancer.The differences of CHI3L1 protein expression was compared in the parameters of sex,age,tumor diameter,cholecystolithiasis,growth pattern,the amount of transferred lymph nodes,distant metastasis,vascular invasion,lymphatic invasion,neural invasion and TNM stage.Results The CHI3L1 protein expression in patients with gallbladder cancer was higher than that in healthy controls (4.45 ± 1.20 vs.0.55 ± 0.94,P ＜ 0.05).Clinic pathological analysis showed that positive CHI3L1 expression was significantly associated with growth pattern,the amount of transferred lymph nodes,distant metastasis and TNM stage (P ＜ 0.05,respectively).Kaplan-Meier Survival Analysis showed that the overall survival was (13.0 ± 7.6) months in CHI3L1 positive patients versus (30.0 ± 6.4) months in CHI3L1 negative patients (P ＜ 0.05).Conclusion The CHI3L1 protein expression in tumor tissue was correlative with prognosis of patients with gallbladder cancer.
