目的 通过有限元方法动态模拟Ni-Ti伞状记忆合金在早期坏死股骨头内打开的过程.方法 应用UG 6.0、Mimics 10.0、Geomagic Studio 11.0、Hypermesh 11.0等软件,采用快速个体化建模方法对早期坏死的股骨头螺旋CT数据进行快速建模;根据记忆合金伞的各个参数,建立Ni-Ti记忆合金伞的三维有限元模型.模拟记忆合金伞通过骨隧道植入股骨头内后打开的过程,采用Lsdyna分析Ni-Ti记忆合金伞打开后的位置,以及股骨头内破坏的情况.结果 记忆合金伞植入过程需要伞的顶部全程固定,才能放到预想位置,有效率为82.4%.患者骨密度直接影响记忆合金伞植入的效果;骨量较低的患者,记忆合金伞穿透股骨头的风险极高.记忆合金伞套筒的位置对放置时产生的股骨头骨质破坏的范围有影响,套筒初始置放于合金伞中部对股骨头骨质破坏最轻.结论 骨密度是Ni-Ti伞状记忆合金植入坏死股骨头手术成败的一个重要影响因素,有助于评估患者手术风险.

Objective To explore dynamic simulation of Ni-Ti memory alloy umbrella necrosis of the femoral head in an open process by the finite element method,which may provide the theoretical support for use of individualized treatment of early osteonecrosis memory alloys.Methods By using fast individual establishment method,the early stage osteonecrosis model of the femoral head was constructed based on data from multi-slice spiral CT with Mimics 10.0,Geomagic Studio 11.0 and Hypermesh 11.0 softwares.According to various parameters of shape memory alloy umbrella,we established a three-dimensional finite element model of Ni-Ti memory alloy umbrella.We simulated memory alloy umbrella commence within the femoral head.Lsdyna was used to analyze Ni-Ti memory alloy position after opening in the femoral head and the destruction of the femoral head.Results The top of umbrella should be fully constrained to avoid the umbrella opening in the unexpected location.The device had an efficacy rate of 82.4%.The patient's bone mineral density (BMD) directly impacted on effect of memory alloy umbrella.The patients with osteoporosis had the high risks that memory alloy umbrella could penetrate the femoral head.With the sleeve placed in the middle,there was none extra damage to the decompression channel.Conclusion BMD is an important factor for success of umbrella memory alloy Ni-Ti implant surgery.It can be used to to asses the risks of the surgery.The proposed finite element analysis method could be used as pre-operation phase of individual treatment for individual patient.
