目的 探讨胰岛素样生长因子-1(IGF-1)在大肠癌患者血清中的表达及其临床意义.方法 采用放射免疫分析法(RIA法)检测32例正常人及62例大肠癌患者手术前血清IGF-1水平,并对42例大肠癌患者手术前后血清IGF-1水平进行比较.结果 血清IGF-1水平与大肠癌患者临床分期、细胞分化程度明显相关,与大肠癌患者性别、年龄无明显相关;大肠癌患者血清IGF-1水平[(255.85 ±56.59) μg/L]明显高于正常对照组[(155.04 ±21.37) μg/L],两者差异有统计学意义(P<0.01);Ⅳ期大肠癌患者血清IGF-1水平[(309.09 ±50.95) 上g/L]明显高于Ⅲ期患者[(274.40±44.47) μg/L]、Ⅲ期患者明显高于Ⅱ期患者[(236.17 ±35.36) μg/L]、Ⅱ期患者明显高于Ⅰ期患者[(201.50 ±38.51) μg/L]、Ⅰ期患者明显高于正常对照组,两者差异有统计学意义(P<0.05);肿瘤细胞分化程度较差(低分化腺癌、印戒细胞癌、黏液腺癌)大肠癌患者血清IGF-1水平[(272.60±53.80) μg/L]明显高于肿瘤细胞分化程度较好(高分化腺癌、中分化腺癌)者[(232.65±52.89) μg/L],两者差异有统计学意义(P<0.05),大肠癌患者术后血清IGF-1水平[(229.83±46.08) μg/L]明显低于术前[(235.24 ±45.98) μg/L],两者差异有统计学意义(P<0.01).结论 血清IGF-1可能与大肠癌的发生发展有关.

Objective To explore the serum expression of insulin like growth factor-1 (IGF-1) in patients with colorectal carcinoma and its clinical significance.Methods The level of serum IGF-1 was measured by radioimmunoassay (RIA) in 32 cases of normal control subjects and 62 cases of colorectal carcinoma,and compared in 42 patients with colorectal carcinoma before and after operation.Results The serum IGF-1 was correlated with with clinical stage and histological grade,but not with sex and age in patients with colorectal carcinoma.The level of serum IGF-1 was elevated in all subgroups of patients with colorectal carcinoma [(255.85 ± 56.59) μg/L] compared to the controls [(155.04 ± 21.37) μg/L] (P <0.01).The serum IGF-1 level in stage Ⅳ [(309.09 ±50.95) μg/L] was higher than that in stage Ⅲ [(274.40 ±44.47) μg/L] (P <0.05),that in stage Ⅲ was higher than that in stage Ⅱ [(236.17 ± 35.36) μg/L] (P <0.05),and in stage Ⅱ it was higher than in stage Ⅰ [(201.50 ±38.51) μg/L] (P < 0.05),and that in stage Ⅰ was higher than that in normal controls (P < 0.01).The levels of serum IGF-1 were significantly higher in the cases of poorly differentiated adenocarcinoma,signet-ring cell carcinoma and mucoid carcinoma [(272.60 ± 53.80) μg/L] than those in the cases of well and moderately differentiated carcinoma [(232.65 ± 52.89) μg/L] (P < 0.05).The serum IGF-1 level was obviously reduced in patients with colorectal carcinoma after operation (P < 0.01).Conclusion These results suggest that serum IGF-1 may be involved in the occurrence and development of colorectal carcinoma.
