目的 探讨硬膜外脊髓电刺激(ESCS)结合减重跑台训练对脊髓损伤大鼠运动功能的影响.方法 共选取成年雌性SD大鼠24只,采用改良Allen打击法将其制作成T8-9脊髓损伤模型,将造模成功大鼠随机分为脊髓损伤组(简称模型组,术后未给予特殊处理),硬膜外电刺激组(简称电刺激组,术后给予硬膜外电刺激)、减重跑台治疗组(简称减重运动组,术后给予减重运动训练)和减重跑台训练结合硬膜外电刺激组(简称治疗组,术后给予减重运动训练及硬膜外电刺激).于术前及术后采用神经行为学评分(BBB评分)对各组实验大鼠运动功能恢复情况进行评定,并于术后8周时取各组大鼠脊髓损伤节段进行神经丝蛋白(NF200)免疫组化染色分析.结果 减重运动组和治疗组大鼠神经行为学评分(BBB评分)和NF200染色光密度值均显著高于模型组及电刺激组水平(P<0.05);治疗组大鼠BBB评分显著高于减重运动组(P<0.05),但治疗组和减重运动组NF200免疫组化染色结果间差异无统计学意义(P>0.05).结论 ESCS及减重运动训练均对不完全性脊髓损伤大鼠步行功能恢复具有促进作用,且两者联用具有协同功效,其治疗机制可能与刺激脊髓损伤下位中枢模式发生器神经元有关.

목적 탐토경막외척수전자격(ESCS)결합감중포태훈련대척수손상대서운동공능적영향.방법 공선취성년자성SD대서24지,채용개량Allen타격법장기제작성T8-9척수손상모형,장조모성공대서수궤분위척수손상조(간칭모형조,술후미급여특수처리),경막외전자격조(간칭전자격조,술후급여경막외전자격)、감중포태치료조(간칭감중운동조,술후급여감중운동훈련)화감중포태훈련결합경막외전자격조(간칭치료조,술후급여감중운동훈련급경막외전자격).우술전급술후채용신경행위학평분(BBB평분)대각조실험대서운동공능회복정황진행평정,병우술후8주시취각조대서척수손상절단진행신경사단백(NF200)면역조화염색분석.결과 감중운동조화치료조대서신경행위학평분(BBB평분)화NF200염색광밀도치균현저고우모형조급전자격조수평(P<0.05);치료조대서BBB평분현저고우감중운동조(P<0.05),단치료조화감중운동조NF200면역조화염색결과간차이무통계학의의(P>0.05).결론 ESCS급감중운동훈련균대불완전성척수손상대서보행공능회복구유촉진작용,차량자련용구유협동공효,기치료궤제가능여자격척수손상하위중추모식발생기신경원유관.
Objective To investigate the potential synergistic effect on motor function recovery of the application of epidural spinal cord stimulation(ESCS)with body weight supported treadmill training after incomplete spinal cord injury.Methods Twenty-four adult,female Sprague-Dawley rats were administered a contusive injury of moderate severity at vertebral level T8~9 using a modification of Allen's method.They were then randomly allocated into 4 groups:a spinal cord injury group(SCI group),a spinal cord injury plus ESCS group(ESCS group),a treadmill training group(treadmill group),and a treadmill training plus ESCS group(treatment group).Before and after surgery,hindlimb functional recovery in the rats was evaluated using the open field Basso,Beattie and Bresnahan (BBB)scale for gait analysis and the expression of neurofilament 200(NF200)was detected by histoehemical staining 8 weeks post surgery. Results Both BBB scores and the expression of NF200 were significantly higher in the treadmill and treatment groups compared with the SCI and ESCS groups.BBB scores in the treatment group were significantly higher than those in the treadmill group.However,no significant difference in terms of NF200 expression was observed between the treatment and treadmill groups. Conclusion The combination of ESCS and treadmill training can yield functional improvement following spinal cord injury.
