目的 探讨青少年特发性脊柱侧凸(AIS)是否对患者跑台运动过程中步态运动学及足底压力分布产生影响.方法 共选取18例青少年特发性脊柱侧凸患者纳入AIS组,同时选取18例脊柱发育正常的同龄青少年纳入正常组.采用三维运动捕捉系统及足底压力鞋垫系统采集2组对象在不同跑台速度(包括6 km/h、8 km/h及10km/h)下的步态运动学及足底压力分布数据.结果 AIS患者在三种速度跑台运动中,其两侧下肢关节在矢状面内的运动及躯干在额状面及横截面内的运动均未出现左右不对称现象.与正常组不同,AIS组患者在运动中其足底压力表现出双侧不平衡现象,以主弯凹侧下肢支撑时的足底最大压力[以6 km/h运动时为(0.128±0.01) N/cm2·kg,以8 km/h运动时为(0.168±0.02)N/cm2·kg,以10 km/h运动时为(0.190±0.03) N/cm2·kg]明显高于以主弯凸侧下肢支撑时的足底最大压力[以6 km/h运动时为(0.115±0.01) N/cm2·kg,以8 km/h运动时为(0.152±0.02) N/cm2·kg,以10 km/h运动时为(0.181±0.03)N/cm2·kg](P<0.05).结论 与步态运动学指标比较,足底压力指标对于评估AIS患者运动对称性具有更佳敏感性.

Objective To investigate the impact of spinal deformity on the gait characteristics and foot pressure distribution of patients with adolescent idiopathic soliosis (AIS) during walking and slow running on the treadmill.Methods Eighteen AIS patients and eighteen age-matched normal adolescents were studied.They walked at 6 km/h and ran slowly at 8 and 10 km/h on a treadmill.Gait kinematics and foot pressure data were collected with a VICON motion capture system and a Medilogic system simultaneously.Results There were no significant bilateral asymmetries in the sagittal lower extremity kinematics of the AIS patients,nor was there abnormal frontal or transverse trunk motion while walking or running.Asymmetries in maximum foot pressure were observed for the AIS patients,with foot pressure values for the convex side significantly less than those observed on the concave side.Conclusion Foot pressure asymmetries are stronger predictors than gait kinematics for AIS patients.
