目的观察冠状动脉(冠脉)支架植入术后在不停用双联抗血小板药物情况下心血管植入型电子器械(CIED)术后并发症发生情况.方法将2008年3月至2013年3月在大连解放军第210医院心内科接受CIED植入的64例患者分为2组,双联抗血小板药物组32例,植入冠脉药物洗脱支架术后1年内持续使用双联抗血小板药物(阿司匹林100 mg+氯吡格雷75 mg,每日1次口服)期间符合CIED适应证,在不停用的情况下植入CIED;对照组32例,采用随机数法抽取同期未使用或停用抗血小板药物情况下植入CIED患者.观察终点:术后30 d内囊袋血肿发生率.结果 2组患者术后30 d内均无囊袋血肿发生.结论冠脉支架植入术后在不停用双联抗血小板药物情况下植入CIED未见囊袋血肿发病率增加,但需增加病例数、延长随访期进一步观察.
목적 관찰관상동맥(관맥)지가식입술후재불정용쌍련항혈소판약물정황하심혈관식입형전자기계(CIED)술후병발증발생정황.방법 장2008년3월지2013년3월재대련해방군제210의원심내과접수CIED식입적64례환자분위2조,쌍련항혈소판약물조32례,식입관맥약물세탈지가술후1년내지속사용쌍련항혈소판약물(아사필림100 mg+록필격뢰75 mg,매일1차구복)기간부합CIED괄응증,재불정용적정황하식입CIED;대조조32례,채용수궤수법추취동기미사용혹정용항혈소판약물정황하식입CIED환자.관찰종점:술후30 d내낭대혈종발생솔.결과 2조환자술후30 d내균무낭대혈종발생.결론 관맥지가식입술후재불정용쌍련항혈소판약물정황하식입CIED미견낭대혈종발병솔증가,단수증가병례수、연장수방기진일보관찰.
Objective To observe the skin-pocket hematoma complication in using dual-antiplatelet drugs after coronary stenting during implantation of cardiovascular implantable electronic device (CIED).Methods This was single-center retrospective study of 32 patients with continue using dualantiplatelet drugs(aspirin,10 mg/d and clopidogrel,75mg/d,Qd) after coronary stenting and another 32 patients with no antiplatelet drugs during implantation of CIED as control.The skin-pocket hematoma complication during 30 days follow-up after operation was compared between two groups.Results There was no statistic difference in skin-pocket hematoma complication during 30 days follow-up after operation between two groups(0％ vs.0％,P＞0.05).Conclusion It maybe safety for the patients with coronary stenting continue using dual-antiplatelet drugs during implantation of CIED and don't increase the skin-pocket hematoma complication.
