目的 观察吸入伊洛前列素治疗肺动脉高压危象的疗效和安全性.方法 本研究入选4例特发性肺动脉高压患者,发生危象时在基础治疗同时,吸入伊洛前列素10μg/次,每次吸入约10 ~ 15 min,每2小时1次,每天8次.观察吸入前后体动脉压、心率和超声心动图各项指标的变化以及患者的不适反应.结果 4例肺动脉高压危象患者吸入伊洛前列素后肺动脉压明显下降,血氧饱和度明显改善.雾化吸入治疗后有2例患者出现恶心、呕吐、腹泻及干咳等不适,其中1例因出现严重呕吐及腹泻而停止治疗.结论 吸入伊洛前列素可以显著降低肺动脉高压危象患者肺动脉压力,改善血氧饱和度.

목적 관찰흡입이락전렬소치료폐동맥고압위상적료효화안전성.방법 본연구입선4례특발성폐동맥고압환자,발생위상시재기출치료동시,흡입이락전렬소10μg/차,매차흡입약10 ~ 15 min,매2소시1차,매천8차.관찰흡입전후체동맥압、심솔화초성심동도각항지표적변화이급환자적불괄반응.결과 4례폐동맥고압위상환자흡입이락전렬소후폐동맥압명현하강,혈양포화도명현개선.무화흡입치료후유2례환자출현악심、구토、복사급간해등불괄,기중1례인출현엄중구토급복사이정지치료.결론 흡입이락전렬소가이현저강저폐동맥고압위상환자폐동맥압력,개선혈양포화도.
Objective To summary the efficacy and safety of aerosolized iloprost in patients with pulmonary hypertensive crisis.Methods On the basis of conventional therapy,aerosolized iloprost (10 μg per time for 10-15 min in 2 hours interval,8 times per day) was administered to four patients with idiopathic pulmonary arterial hypertension and pulmonary hypertensive crisis.Blood pressure,heart rate,systemic artery oxygen saturation,systolic pulmonary arterial pressure (sPAP) measured by echocardiography and the adverse events were analyzed.Results After aerosolized iloprost therapy,sPAP was significantly decreased and systemic artery oxygen saturation was improved.Adverse events (nausea,vomiting,diarrhea,dry cough) were observed in two patients,and the iloprost use was stopped in one patient due to severe vomiting and diarrhea.Conclusion Aerosolized iloprost could significantly reduce the sPAP and improve the systemic artery oxygen saturation in patients with pulmonary hypertension crisis.
