目的 无创快速评估小婴儿室间隔缺损(VSD)修补术后早期肌力指数的改变,指导临床合理用药.方法 采用超声心输出量监测仪(USCOM)观察VSD修补术后1~2周小婴儿的血流动力学指标,以同龄健康小婴儿为对照.结果 两组小婴儿各50例,月龄中位数为3个月,VSD术后组的肌力指数(SMII)明显高于健康对照组[(1.4±0.4)对(1.1±0.2)W/m2,P<0.01];每搏指数和每公斤心输出量均较健康对照组明显偏高(P<0.05),收缩期血液流动时间较健康对照组明显缩短(P<0.01).结论 小婴儿VSD修补术后早期,循环功能各项指标尚未完全恢复至正常水平,有待后期随访监测.肌力指数直接反映心肌功能,可能在术后随访用药中有重要的临床指导意义.

Objective To evaluate inotropy index(SMII) in infants early after surgical repair of ventricular septal defect (VSD),which could guide rational clinical treatment.Methods This was a prospective and observational study.Subjects were infants 1-2 weeks after VSD repair,along with age-matched healthy infants as the control group.Hemodynamic parameters were measured using ultrasonic cardiac output monitor(USCOM),respectively.Results There were 50 subjects in both groups,representing the same age distribution 1 and 6 (median 3) months.In VSD postoperative group,the SMII was significantly higher than the control group [(1.4 ± 0.4) vs.(1.1 ± 0.2) W/m2,P < 0.01],stroke index and cardiac output per kilogram were significantly higher than those in control group (P < 0.05),and systolic blood flow time was significantly shorter than the control group (P < 0.01).Conclusion For infants undergone VSD repair,the circulatory function was not fully recovered to normal level at early postoperation,pending long-term follow-up.Inotropy index directly reflects myocardial function,may play an important role in guiding medication in the postoperative follow-up.
