目的探讨应用全厚板层角膜移植术(DLK)治疗病毒性角膜炎所致的角膜斑翳的临床效果,并评价其安全性.方法回顾2009年1月至2011年6月浙江省宁波市鄞州人民医院眼科中心临床确诊为病毒性角膜炎所导致的角膜斑翳(病灶均非全层)16例(16眼),常规抗病毒滴眼液治疗后,确定角膜炎症已消退或处于静止期,行全厚板层角膜移植术,观察术后角膜植片透明率及角膜植片免疫排斥反应等情况.结果本组患者治愈16例,成功率为100％;随访观察18 ～ 24个月,术后角膜植片全部透明,矫正视力为0.2～0.6,随访期间无复发,术后早期存在2例排异反应,经治疗后控制,全部16例患者在长期随访中未出现排异反应.结论全厚板层角膜移植术治疗病毒性角膜炎所致角膜斑翳可显著提高视力,并且并发症少,是一种安全、有效的治疗方法.

Objective To explore the clinical safety and efficacy of deep lamellar keratoplasty for corneal macula after viral kerititis.Methods A total of 16 patients (16 eyes) of corneal macula after viral kerititis at our center from January 2009 to June 2011 received anti-virus eye-drops and then underwent deep lamellar keratoplasty when inflammation were dissolved or quiescent.And the corneal graft diaphaneity and corneal graft rejection reactions were observed.Results All 16 patients were recovered with a success rate of 100％.After a follow-up of 18-24 months,all corneal grafts were transparent and best-corrected visual acuity was 0.2-0.6.There was no reoccurrence during the follow-up period.After a follow-up of several months,2 of them had rejection reaction and were controlled after treatment.No rejection reaction occurred during the long-term follow-up.Conclusion As a safe and efficacious procedure, deep lamellar keratoplasty can significantly raise vision acuity with few complications.
