目的 探讨磁共振波谱成像(1H-MRS)对肝纤维化早期诊断、量化分析及其分期的价值.方法 CCl4法建立大鼠肝纤维化模型,成模大鼠40只及对照组7只进行MR波谱成像,成模大鼠按肝纤维化病理分期进行分组,分别计算并比较各组代谢物[胆碱(Cho)、谷氨酰胺及谷氨酸复合物(Glx)、乳酸(Lac)、肌酸(Cr)]与脂质(Lip)的峰高、波峰下面积的比值.结果 (1)代谢物与脂质波峰峰高比值比较:Cho/Lip比值:对照组(0.05±0.03)与3期(0.40±0.30)、4期(0.44-0.30)比较P <0.05;Glx/Lip比值:对照组(0.15 ±0.13)与2期(0.66±0.55)、3期(0.75±0.45)、4期(0.76±0.52)比较P<0.05;Cr/Lip比值:对照组(0.14 ±0.27)与3期(0.92 ±0.58)比较P<0.05.(2)代谢物与脂质波峰下面积比值比较:Cho/Lip比值:对照组(0.12±0.13)与4期(0.47±0.37)比较P<0.05;Glx/Lip比值:对照组(0.05±0.04)与1期(0.54±0.32)、2期(0.45±0.36)、3期(0.48±0.40)、4期(0.53±0.34)比较P<0.05; Cr/Lip比值:对照组(0.11±0.23)与4期(0.81±0.71)比较P <0.05.各组Lac/Lip波峰峰高及面积比值比较P>0.05.结论 Cho/Lip、Glx/Lip、Cr/Lip波峰峰高比值、波峰下面积比值对肝纤维化分期具有一定的价值.

Objective To explore the values of magnetic resonance spectrum (MRS) in early diagnosis,quantization analysis and staging of hepatic fibrosis.Methods A rat model of hepatic fibrosis was established by the method of carbon tetracarbon (CCl4).A total of 47 SD rats were divided into model (n =40) and control (n =7) groups.1H-MRS was performed.The model rats of hepatic fibrosis were grouped according to their pathological stages.The ratio of peak height and peak area of metabolites and lipid (Cho/Lip,Glx/Lip,Lac/Lip and Cr/Lip) were calculated and compared respectively.Results The ratios of peak height of metabolites and lipid were as follows:ratio of Cho and Lip:significant differences existed between control and grades 3 and 4 model groups (P < 0.05) ; ratio of Glx and Lip:significant differences existed between control and grades 2,3 and 4 model groups (P < 0.05) ; ratio of Cr and Lip:significant differences existed between control and grade 3 model groups (P < 0.05).The peak area ratio of main metabolites and lipid of liver were as follows:ratio of Cho and Lip:significant differences existed between control and grade 4 model groups (P < 0.05) ; ratio of Glx and Lip:significant differences existed between control and other groups (P < 0.05) ; ratio of Cr and Lip:significant differences existed between control and grade 4 model groups (P < 0.05) ; ratio of Lac and Lip:no significant differences existed between these groups (P >0.05).Conclusion The ratios of peak height and peak area of Cho/Lip,Glx/Lip and Cr/Lip are important for the staging of hepatic fibrosis.
