目的探讨胃癌患者外周血T淋巴细胞、单核细胞的表面分子变化及Th1/Th2的漂移,并分析肿瘤负荷对胃癌患者的免疫功能的影响.方法选取胃癌患者30例及同期体检的健康志愿者30例,采用流式细胞仪检测和分析外周血T淋巴细胞、单核细胞的表面分子及Th1/Th2细胞因子的变化.结果 T淋巴细胞中CD3+、CD4+28+/CD3+、CD8+28+/CD3+水平均低于对照组(P＜0.01);CD8+3+/CD28-较正常组升高(P＜0.01);单核细胞的表面分子CD40与正常对照组相比差异无统计学意义(P＞0.05),但是CDso与CD86均相对于正常水平降低(P＜0.01); Th1细胞因子(IL-2、IFN-γ)降低(P＜ 0.01); Th2细胞因子(IL-4、IL-10)的表达较正常组升高(P＜0.01).结论胃癌患者的免疫功能受损,胃癌的发病可能与免疫功能低下有一定关系;Th1/Th2失衡,表明肿瘤细胞出现免疫逃避与胃癌的发展可能也有关系.
목적 탐토위암환자외주혈T림파세포、단핵세포적표면분자변화급Th1/Th2적표이,병분석종류부하대위암환자적면역공능적영향.방법 선취위암환자30례급동기체검적건강지원자30례,채용류식세포의검측화분석외주혈T림파세포、단핵세포적표면분자급Th1/Th2세포인자적변화.결과 T림파세포중CD3+、CD4+28+/CD3+、CD8+28+/CD3+수평균저우대조조(P＜0.01);CD8+3+/CD28-교정상조승고(P＜0.01);단핵세포적표면분자CD40여정상대조조상비차이무통계학의의(P＞0.05),단시CDso여CD86균상대우정상수평강저(P＜0.01); Th1세포인자(IL-2、IFN-γ)강저(P＜ 0.01); Th2세포인자(IL-4、IL-10)적표체교정상조승고(P＜0.01).결론 위암환자적면역공능수손,위암적발병가능여면역공능저하유일정관계;Th1/Th2실형,표명종류세포출현면역도피여위암적발전가능야유관계.
Objective To investigate the changes of surface molecules in T lymphocyte,mononuclear and the shift of Th1/Th2 in patients with gastric cancer and to analyze the influence of the tumor load on the immunity function with gastric cancer.Methods Peripheral blood samples were collected from 30 preoperative patients with gastric cancer and 30 healthy controls.The changes of surface molecules in T lymphocyte,mononuclear and the state of Th1/Th2 were investigated and analyzed by flow cytometry.Results The levels of CD3+、CD4+28+/CD3+、CD8+28+/CD3+ were lower than those in control group (P ＜ 0.01); CD8+3+/CD28-was higher than that of the normal group (P ＜ 0.01); the mononuclear surface molecules CD40 was not significantly different with that of normal group (P ＞ 0.05),but the CD80 and CD86 were all lower than those of the normal (P ＜ 0.01); Th1 cytokines (IL-2,IFN-γ) were all lower (P ＜ 0.01); Th2 cytokines (IL-4,IL-10) were all higher than those in the normal (P ＜ 0.01).Conclusion The cellular immunity of patients with gastric cancer was inhibited,which maybe related to the occurrence of gastric carcinoma; Th1/Th2 was out of balance,the result indicated the immune escape of tumor cell maybe also relevant with the development of gastric cancer.
