目的探讨自动化梅毒螺旋体抗体初筛实验的特异性并进行流程改进分析.方法收集2012年11月北京协和医院患者血清标本4690份.采用化学发光微粒子免疫分析法(CMIA)对4690份血清标本进行梅毒血清学初筛,阳性标本进行梅毒螺旋体颗粒凝集试验(TPPA)复检并同时行梅毒快速血浆反应素环状卡片试验(RPR).CMIA初筛与TPPA复检不符的标本进行荧光螺旋体抗体吸收试验(FTA-ABS)验证.结果 4690份血清标本经CMIA初筛96份阳性(2.04％).在96份标本中,TPPA复检阳性仅67份(69.79％),RPR检测41份阳性(42.71％).29份标本CMIA阳性而TPPA阴性的标本,FTA-ABS验证结果IgM全部阴性,3份标本IgG抗体可疑阳性、26份IgG抗体阴性.结论 CMIA进行梅毒血清学筛查时存在假阳性问题,需要改进筛查流程.

Objective To analyze the specificity of the automated platform using treponemal test for initial screening and discuss the improvement of the screening procedure.Methods Totally 4690 serum specimens were collected from Peking Union Medical College Hospital in November 2011.The automated chemiluminescence micro particle immunoassay (CMIA) was used for initial screening of treponemal antibodies in 4690 serum specimens,among which the positive reaction specimens were re-tested by treponema pallidum particle agglutination assay (TPPA) and syphilis rapid plasma regain test (RPR).Fluorescent treponemal antibody absorption test (FTA-ABS) was used for confirmation in specimens with discordant results between CMIA and TPPA.Results Of 4690 serum specimens for syphilis serology screening,96 (2.04％)were initially tested positive with CMIA.Out of the 96 specimens,only 67 were retested positive with TPPA and 41 were tested positive with RPR.In 29 specimens with discordant results between CMIA and TPPA,3 were found with doubtful positive of treponemal IgG antibodies and no treponemal IgM antibodies were found by using FTA-ABS.Conclusion False positive results were found in initial screening of treponemal test with CMIA and improvement for screening procedure was needed.
