目的 通过对半侧颜面短小畸形(Hemifacial microsomia,HFM)患儿手术前、后头颅正位X线片及三维重建图像进行测量,研究下颌骨牵引治疗对上颌骨发育的影响.方法 测量22例患者术前(T1)和术后半年(T2)头颅正位X线片上颌咬合平面及鼻基底平面与眶下平面的夹角,取相同的时间点在15例患者三维重建图像上测量第5齿颊侧牙槽嵴顶中点,及上颌窦底最低点与参考平面的距离,应用统计学方法进行分析处理.结果 上颌咬合平面与眶下平面的夹角术前、后比较,差异有统计学意义(P<0.05);鼻基底平面与眶下平面的夹角术前、后比较,差异无统计学意义(P>0.05).三维测量正常侧上颌窦最低点无显著变化,患侧上颌窦最低点和两侧牙槽嵴顶中点均发生显著下降.结论 下颌骨牵引治疗可以促进患儿上颌骨的发育,生长的部分包括牙槽骨段及上颌窦.

목적 통과대반측안면단소기형(Hemifacial microsomia,HFM)환인수술전、후두로정위X선편급삼유중건도상진행측량,연구하합골견인치료대상합골발육적영향.방법 측량22례환자술전(T1)화술후반년(T2)두로정위X선편상합교합평면급비기저평면여광하평면적협각,취상동적시간점재15례환자삼유중건도상상측량제5치협측아조척정중점,급상합두저최저점여삼고평면적거리,응용통계학방법진행분석처리.결과 상합교합평면여광하평면적협각술전、후비교,차이유통계학의의(P<0.05);비기저평면여광하평면적협각술전、후비교,차이무통계학의의(P>0.05).삼유측량정상측상합두최저점무현저변화,환측상합두최저점화량측아조척정중점균발생현저하강.결론 하합골견인치료가이촉진환인상합골적발육,생장적부분포괄아조골단급상합두.
Objective To analyze the effect of mandibular distraction on the maxilla growth in children with hemifacial microsomia through measurement with the posterior-anterior cephalometric X-ray films and Three-dimensional CT reconstruction images.Methods The deviation angular of maxilla occlusion plane and nasal base plane from the infra-orbital plane were measured on the posterior-anterior cephalometric X-ray films in 22 patients before and half a year after operation.The vertical distance from the midpoint of 5th teeth alveolar and the lowest point of maxillary sinus to reference plane were measured on 3D reconstruction images in 15 patients.The data were statistically analyzed.Results On posterioranterior cephalometric X-ray films,the cant of occlusion plane were significantly reduced (P < 0.05),While the angular of nasal base plane and the infra-orbital plane had no significant change.On 3D reconstruction images,all the detection points had significantly declined except the lowest point of maxillary sinus on normal side.Conclusions Distraction osteogenesis of mandible can promote the growth of the maxilla in children with HFM,the accelerated growth parts include alveolar bone and maxillary sinus.
