目的评价体部伽马刀在肾上腺转移癌中的疗效与安全性.方法回顾分析采用体部伽马刀治疗的24例肾上腺转移癌患者临床资料,共26个病灶.计划要求50％的等剂量线覆盖100％计划靶体积,单次照射剂量3～5Gy,5次/周,治疗10～17次,总剂量40～52 Gy.结果中位随访时间28个月,随访率为100％.病灶完全缓解率、部分缓解率分别为53.8％(14/26)、30.8％(8/26),总有效率为84.6％.1、2年局部控制率分别为100.0％(15/15)、90.0％(9/10).治疗后1、2、3年生存率分别为62.5％(15/24)、41.7％(10/24)、18.2％(4/22).治疗前腰背痛6例,治疗后疼痛完全缓解2例,疼痛减轻3例.不良反应多为Ⅰ～Ⅱ级消化道及血液学反应.结论体部伽马刀治疗肾上腺转移癌疗效好,不良反应轻.

Objective To evaluate clinical efficacy and safety of gamma-knife in the treatment of adrenal metastases.Methods The data from 24 patients with adrenal metastatic carcinoma performed with γ-SBRT were analyzed.A total dose sent 3-5 Gy/fiaction to 50 ％ isodose covering 100 ％ PTV,the radiation dose to adrenal nidus ranged from 40 to 52 Gy,and five times per week.Results The median follow-up time was 28 months,and the follow-up rate was 100 ％.There were 26 lesions.The local lesions response rate was 84.6 ％ (22/26),with complete remission was 53.8 ％ (14/26) and partial was 30.8 ％ (8/26).Local control rate of 1-year,2-year were 100.0 ％ (15/15) and 90.0 ％ (9/10).The overall survival rate of 1-year,2-year and 3-year were 62.5 ％ (15/24),41.7 ％ (10/24) and 18.2 ％ (4/22).6 cases had back pain,2 patients had complete relief of pain,3 patients had pain relief.The majority of radioreactions were Grades 1 to 2 toxicities.Conclusion Gamma knife treatment has yielded good curative effect on adrenal metastatic carcinoma,and adverse reactions are mild.
