目的探讨踝关节镜辅助下的关节腔探查清理术对踝关节痛风性关节炎的诊断和治疗意义.方法自2007年9月至2011年8月,对16例踝关节痛风性关节炎患者行关节镜检,使用低温等离子汽化刀及刮匙除去关节软骨表面的尿酸盐结晶,刨刀刨削清理增生的滑膜,修整剥脱的关节软骨；最后用大量生理盐水冲洗关节腔,术后常规降尿酸治疗,早期关节功能锻炼.结果 16例患者经过3～18个月术后随访,未出现急性痛风性关节炎症状,术后踝关节功能得到恢复.结论关节镜清理关节腔内痛风结晶及炎性滑膜治疗踝关节痛风性关节炎,创伤小,恢复快,疗效确切.
목적 탐토과관절경보조하적관절강탐사청리술대과관절통풍성관절염적진단화치료의의.방법 자2007년9월지2011년8월,대16례과관절통풍성관절염환자행관절경검,사용저온등리자기화도급괄시제거관절연골표면적뇨산염결정,포도포삭청리증생적활막,수정박탈적관절연골；최후용대량생리염수충세관절강,술후상규강뇨산치료,조기관절공능단련.결과 16례환자경과3～18개월술후수방,미출현급성통풍성관절염증상,술후과관절공능득도회복.결론 관절경청리관절강내통풍결정급염성활막치료과관절통풍성관절염,창상소,회복쾌,료효학절.
Objective To investigate the application of debridement under arthroscopy in the diagnosis and treatment of gout arthritis in ankle.Methods A total of 16 patients with gouty arthritis in ankle were diagnosed by arthroscopy between September 2007 and August 2011.The urate crystal stones deposited on the surface of cartilage were removed by radiofrequency evaporating apparatus and curettes.The hyperplastic synovium and the urate crystal deposited on it were cut by the shavers under arthroscopy.The joint debridement,cartilage repair were done at the same time.Irrigation of normal saline was used before the end of the surgery.Diet and drug therapy were continued after the operation.Postoperative joint function exercise were emphasized.Results Acute gouty arthritis did not occur in 16 patients in 3 months to 18 months follow-up.The ankle functions of 16 patients were improved.Conclusions The treatment of gout arthritis in ankle with debridement under arthroscopy has advantages of small incision and quicker recovery was also an effective method.
