目的 探讨纳洛酮联合银杏达莫对肺源性心脏病急性加重期患者血浆脑钠肽(BNP)和内皮素-1(ET-1)的影响.方法 80例肺源性心脏病急性加重期患者随机分为治疗组40例和对照组40例,对照组采用一般常规治疗,治疗组在常规治疗的基础上加用纳洛酮和银杏达莫,两组共治疗10 d.比较治疗前后两组患者的动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)和静息状态下平均肺动脉压(mPAP);比较两组患者BNP和ET-1浓度变化.结果 两组治疗后PaO2均有升高,治疗组升高幅度优于对照组,差异有统计学意义(P<0.05);两组PaCO2均有下降,治疗组优于对照组(P<0.05);两组治疗后mPAP均有下降,且治疗组降低幅度优于对照组,差异有统计学意义(P<0.05);两组治疗后都可明显降低血浆BNP和ET-1的含量,但治疗组降低程度优于对照组(P<0.05).结论 纳洛酮联合银杏达莫改善肺源性心脏病急性加重期患者的病情,可能与改善患者的BNP和ET-1有关.

목적 탐토납락동연합은행체막대폐원성심장병급성가중기환자혈장뇌납태(BNP)화내피소-1(ET-1)적영향.방법 80례폐원성심장병급성가중기환자수궤분위치료조40례화대조조40례,대조조채용일반상규치료,치료조재상규치료적기출상가용납락동화은행체막,량조공치료10 d.비교치료전후량조환자적동맥혈양분압(PaO2)、동맥혈이양화탄분압(PaCO2)화정식상태하평균폐동맥압(mPAP);비교량조환자BNP화ET-1농도변화.결과 량조치료후PaO2균유승고,치료조승고폭도우우대조조,차이유통계학의의(P<0.05);량조PaCO2균유하강,치료조우우대조조(P<0.05);량조치료후mPAP균유하강,차치료조강저폭도우우대조조,차이유통계학의의(P<0.05);량조치료후도가명현강저혈장BNP화ET-1적함량,단치료조강저정도우우대조조(P<0.05).결론 납락동연합은행체막개선폐원성심장병급성가중기환자적병정,가능여개선환자적BNP화ET-1유관.
Objective To investigate the effect of naloxone combined with ginkgo bilobate extract on the brain natriuretic peptide(BNP)and endothelin-1 (ET-1) in the plasma of patients with pulmonary heart disease.Methods Eighty patients with pulmonary heart disease were randomly divided into two groups,the control group (n =40) and the treatment group (n =40).Patients in the control group were treated with conventional treatment,while patients in the treatment group were additionaly given naloxone combined with ginkgo bilobate extract.The subjects were treated for ten days.Results Compared with the control group,the PaO2 was significantly improved (P < 0.05).PaCO2 and mPAP were more significantly reduced in the treatment group than those in the control group (P <0.05).The plasma levels of BNP and ET-1 were significantly decreased after treatment in both groups,especially in the treatment group (P < 0.05).Conclusions Naloxone combined with ginkgo bilobate extract could improve the conditions of pulmonary heart disease,which are related with the reduction of BNP and ET-1.
