目的 探讨玻璃化冷冻法和程序化冷冻法与复苏后卵裂球损伤胚胎的移植周期临床结局的关系.方法 回顾性分析257个玻璃化冷冻周期和152个程序化冷冻周期,均解冻2枚胚胎并移植,根据移植胚胎卵裂球损伤情况分为完全存活组(A组)、1枚胚胎卵裂球损伤组(B组)、2枚胚胎卵裂球均损伤组(C组),比较两种冷冻方法复苏后胚胎完全存活率,以及卵裂球损伤胚胎移植周期的妊娠率、种植率、早期流产率等指标.结果 在均解冻并移植2枚胚胎的情况下,玻璃化冷冻法组复苏后胚胎完全存活率显著高于程序化冷冻法组(78.79% vs40.46%,P<0.01),但在移植有1枚胚胎卵裂球损伤组妊娠率(47.61% vs 54.39%,P>0.05)、种植率(29.36%vs 38.60%,P>0.05)、早期流产率(20.00% vs 16.13%,P>0.05)比较差异无统计学意义;移植有2枚胚胎卵裂球均损伤组妊娠率(30.43% vs 33.87%,P>0.05)、种植率(19.56% vs 23.39%,P>0.05)、早期流产率(28.57% vs 9.52%,P>0.05)比较差异无统计学意义.结论 玻璃化冷冻法与程序化冷冻法相比,能显著提高胚胎复苏的完全存活率,但对复苏后有卵裂球损伤胚胎的移植周期临床结局没有影响.

Objective To compare the clinical outcome of different freezing methods (vitrification and programmed-freezing) and survival state of blastomere cells after frozen-thawed.Methods A retrospective study was performed by analyzing 257 cycles and 152 cycles,which freezing embryos with vitrification and programmed-freezing.Both numbers of embryos thawed and transferred of all cycles were 2 embryos.According to the survival state of blatomeres after frozen-thawed,both groups vitrification and programmed-freezing were divided into 3 subgroups:subgroup A,both embryos were intact; subgroup B,one embryo was damaged;subgroup C,both embryos were damaged.Intact embryos rate,pregnancy rate,implantation rate,early abortion rate was compared.Results The difference in intact embryo rate between programmed-freezing and vitrification was significantly(78.79% vs 40.46%,P < 0.01).There were no statistically significant in the pregnancy rate(47.61% vs 54.39%,P > 0.05) (30.43% vs 33.87%,P > 0.05),implantation rate(29.36% vs 38.60%,P > 0.05) (19.56% vs 23.39%,P > 0.05) and early abortion rate(20.00% vs 16.13%,P > 0.05)(28.57% vs 9.52%,P > 0.05) of subgroup B and C between the two freezing methods.Conclusions Compare to programmed-freezing,vitrification is a method that could improve intact embryos rate after frozen-thawed.However,there is no significant difference in outcomes of blastomere damaged embryos transferred cycles.
