目的 比较脐血、成人外周血淋巴细胞分化(CD)抗原含量的异同,分析进行脐血和成人外周血造血干细胞移植(HSCT)时的利弊,发生移植物抗宿主病(GVHD)和移植物抗白血病(GVL)效应强弱的可能原因.方法 应用流式细胞术(FCM),双色、单色荧光素标记单克隆抗体(McAb)分别检测脐血、成人外周血中淋巴细胞分化抗原,并进行比较分析.结果 成人外周血中相对成熟的总T淋巴细胞(CD3+细胞)、细胞毒T细胞(CD3+、CD8+细胞)、NK细胞(CD16+ CD56+细胞)的含量均明显高于脐血(P<0.01),而B淋巴细胞(CD19+细胞)的含量明显低于脐血(P <0.001).辅助性T淋巴细胞(CD3+、CD4+细胞)、CD4+/CD8+细胞比值脐血与外周血相比,两者比较差异无统计学意义(P>0.05).代表初始T细胞重要表型CD4+、CD45RA+淋巴细胞的含量脐血明显多于成人外周血(P<0.01),而CD8+、CD45 RA+淋巴细胞在脐血中的百分含量略高于外周血,两者比较差异无统计学意义(P>0.05),代表记忆生T细胞重要表型CD45RO+淋巴细胞的含量成人外周血明显多于脐血(P<0.01).结论 脐血中部分淋巴细胞亚群分化抗原的表达能力与成人外周血存在不同程度的差异.脐血中相对成熟的总T淋巴细胞、细胞毒T细胞、NK细胞的含量均低于成人外周血,相对“初始化”淋巴细胞(CD45RA+细胞)含量、B淋巴细胞、CD4+/CD8+细胞比值高于外周血,这些可能与脐血造血干细胞移植后急、慢性GVHD发生率低、强度弱,GVL效应好相关.

Objective To explore the percentage content of cluster of differentiation antigen about lymphocyte from umbilical cord blood and adult peripheral blood,and to analyse the advantages and disadvantages of CB and PB hemopoietic stem cell transplantation (CB/PBHSCT) and the possible mechanism of graft versus host disease (GVHD) and graft versus leukemia (GVL).Methods Adopted flow cytometry(FCM),double or single fluorescein marked monoclone antibody (McAb) to detect lymphocyte CD antigen in CB and PB and to perform comparison and analysis.Results The contents of relatively mature T lymphocyte (CD3 + cell),cytotoxic T lymphocyte (CD3 + CD8 + cell),natural killer cell (NK cell,CD16 + CD56 + cell) in PB were significantly higher than those in CB (P < 0.01),while B lymphocyte(CD19+ cell)in PB were lower than those in CB(P <0.01).There was no significant defference between the ratios of helper T lymphocyte (CD3 + CD4 + cell) and CD4 +/CD8 + cell compared with in CB and in PB (P > 0.05).The content of lymphocyte representing the major phenotype CD4+ CD45RA+ lymphocyte of naive T cell in CB was significantly higher than those in PB(P < 0.01),but there was no significant difference between them about CD8 + CD45RA+ lymphocyte (P > 0.05).The content of lymphocyte representing the major phenotype CD45RO+ lymphocyte of memory T cell in PB were significantly higher than those in CB (P < 0.01).Conclusions The expressive ability of CD antigen of subgroups of lymphocyte in CB is different in some degree from that in PB.Relatively mature T lymphocyte,NK cells and lymphocyte in CB secrete cytokines were less than those in PB,compare with "initialized" lymphocyte (CD45RA + cell),B lymphocyte,CD4 +/CD8 + cell ratio is higher in CB than those in PB,which are associated with low incidence and inferior intensity of acute or chronic GVHD and well GVL effect after CBHSCT.
