目的 探讨肺部不同病原体感染时降钙素原(PCT)及C-反应蛋白(CRP)的血清水平.方法 收集肺部感染患者116例,分为革兰阴性菌组(42例)、革兰阳性菌组(39例)及对照组(35例),比较三组血清PCT和CRP水平.结果 革兰阴性菌组[(18.49±7.92) ng/ml]、革兰阳性菌组[(5.36±3.04) ng/ml]及对照组[(0.26±0.13) ng/ml],PCT水平比较差异有统计学意义(P<0.05),且革兰阴性菌组PCT水平高于革兰阳性菌组及对照组,革兰阳性菌组的PCT水平高于对照组;三组患者的CRP水平[(87±48)mg/L、(66 ±49) mg/L、(52±34) mg/L]均增高,差异无统计学意义(P>0.05).结论 降钙素原可用来区分肺部革兰阴性菌及革兰阳性菌感染,而C-反应蛋白特异性较低.

Objective To investigate the value of procalcitonin and C-reactive protein on distinguishing gram-negative bacteria and gram-positive bacteria in pulmonary infection.Methods Serum PCT and CRP levels were measured and compared in patients with pulmonary infection in RICU.Amorg all the patients,42 cases were infected with gram-negative bacteria,39 cases were infected with gram-positive bacteria,and another 35 cases as a control g oup.Results The PCT levels of gram-negative bacteria were higher than those of gram-positive bacteria and the controls,the PCT levels in patients with gram-positive bacteria were higher than that in the control group (P < 0.05).The CRP levels of the three groups were increased similarly(P > 0.05).Conclusions PCT could be used to distinguish gram-negative bacteria and gram-positive bacteria in pulmonary infection.And the specificity of CRP was low.
