目的 探讨酒精性心肌病(ACM)的临床特征、诊断和治疗.方法 对46例ACM患者的临床资料进行回顾性分析.结果 ACM以胸闷、心悸、气促、端坐呼吸、阵发性夜间呼吸困难为主要表现,心电图可表现为多种心律失常,均无特异性；胸部X线均有心脏扩大,心影明显增大呈球形及心胸比＞60％常见；心脏超声心动图检查主要表现为“一大二小”,心脏扩大和左室射血分数减低,心脏超声心动图对诊断、治疗效果的观察、预后的评价均具有非常重要的意义,可作为临床确诊和复查的首选方法.ACM治疗的关键是戒酒,戒酒治疗后患者心脏缩小,左室射血分数增加,心功能改善.结论 应提高对ACM的认识,以早期诊断,及时治疗.

Objective To investigate the clinical features,diagnosis and treatment of alcoholic cardiomyopathy (ACM).Methods The clinical data of 46 cases of alcoholic cardiomyopathy were retrospectively analyzed.Results The cardinal symptoms of alcoholic cardiomyopathy included chest tightness,palpitation,shortness of breath,orthopnea,paroxysmal nocturnal dyspnea.The electrocardiogram could be expressed in a variety of cardiac arrhythmias.Both of them had no specificity ; Chest X-ray all had the enlargement of the heart.The heart shadow was increased significantly as spherical,the cardiothoracic ratio ＞ 60％,both of them were common; The main performance of echocardiography was "one large two small",which included cardiac enlargement and reduced left ventricular ejection fraction.Echocardiography is important to diagnose the disease,observe the therapeutic efficacy,and evaluate the prognosis,which can be used as the preferred method of clinical diagnosis and review.Alcohol withdrawal is essential to the treatment of the disease.Heart bridging,left ventricular ejection fraction increased and heart function improving were found after alcohol withdrawal treatment.Conclusions In order to early diagnosis and treatment,we should improve the understanding of alcoholic cardiomyopathy.
