目的观察常规治疗与联合抗抑郁焦虑治疗在河南籍老年反流性食管病(GERD)疗效中的性别差异.方法将160例合并抑郁/焦虑的老年NERD患者随机分为常规组(兰索拉唑、伊托必利)和治疗组(兰索拉唑、伊托必利+心理疏导、黛力新),疗程均为4周.结果常规组与治疗组疗效比较差异有统计学意义(x2=14.462,P=0.000143 ＜0.05),常规组男与女疗效比较差异无统计学意义(x2=0.216,P=0.642),治疗组男与女疗效比较差异有统计学意义(x2=8.193,P=0.004).结论老年NERD患者在PPI联合促胃动力药标准治疗的基础上加用心理疏导、抗抑郁焦虑药物能显著改善临床症状,且女性患者的疗效优于男性患者.

Objective To observe the gender difference between efficacy of conventional treatment and joint antidepressant,antianxiety treatment on elderly patients with non-erosive reflux disease (NERD).Methods A total of 160 NERD complicated with psychologic obstacle elderly patients were randomly divided into normal group (lansoprazole,Itopride) and treatment group (lansoprazole,Itopride + psychological counseling,Deanxit).The clinical outcomes of the two groups were compared after 4 weeks.Results There was significant diffierence in effect between the treatment group and the normal group(x2 =14.462,P =0.000143 ＜ 0.05).In the treatment group,the difference of total effective rate between female and male was statistically significant (x2 =8.193,P =0.004),while that was not statistically significant (x2 =0.21,P =0.642) in the normal group.Conclusions Psychological counseling joint with antidepressant and antianxiety drugs can significantly ameliorate the clinical symptoms on the basis of standard treatment with PPI and prokinetic drugs for elderly patients with NERD,and the effect on female patients is superior to that on male patients.
