目的 研究C-反应蛋白(CRP)、血糖(Glu)、三酰甘油(TG)在急性胰腺炎(AP)急性期的诊断、病情评估及预后评价的临床应用价值.方法 将234例急性胰腺炎患者分为轻症胰腺炎(MAP)组156例及重症胰腺炎(SAP)组78例,69例健康体检人员为对照组,比较各组CRP、Glu、TG的检测情况.结果 SAP组、MAP组及对照组CRP、Glu、TG组间比较差异均有统计学意义(P＜0.05);CRP、Glu均在疾病的第3天达峰值;AP患者中CRP、Glu、TG与Ranson评分、APACHE-Ⅱ评分均呈正相关(P＜0.05),其中以CRP、Glu相关系数较高.结论 CRP、Glu、TG在急性胰腺炎中均出现不同程度的升高,且与病情的严重程度相关,对CRP、Glu、TG进行监测,在AP患者的诊断、病情评估及预后评价等方面均有重要的临床应用价值.

Objective To study the clinical valve of C-reactive protein (CRP),blood glucose (Glu) and triglyceride(TG) on the diagnosis,severity evaluation and prognosis evaluation of acute pancreatitis(AP) value.Methods Two hundred and thirty-four cases of acute pancreatitis were divided into mild pancreatitis group (MAP) (156 cases) and severe acute pancreatitis group (SAP) (78 cases),69healthy persons were chose as control group.The levels of CRP,Glu,TG were compared.Results CRP,Glu,TG in SAP group,MAP and control groups showed statistical differences (P ＜ 0.05) ; CRP,Glu reached its peak in the first days of the disease ; CRP,Glu,TG and Ransonscore,APACHE-Ⅱ score of AP patients showed positive correlation(P ＜ 0.05),in which the correlation coefficient of CRP and Glu was high.Conclusions For patients with acute pancreatitis,CRP,Glu,TG shows varying degrees of elevated and associated with the severity of the disease.Monitoring of CRP,Glu,TG have important clinical value on the diagnosis,condition assessment and prognostic evaluation of AP.
