目的 探讨腹腔镜胆囊切除术对胆结石患者超敏C-反应蛋白(hs-CRP)和胰岛素抵抗的影响.方法 将106例手术治疗的胆结石患者随机分为观察组和对照组,每组53例,观察组采用腹腔镜胆囊切除术,对照组采用小切口胆囊切除术,测定两组患者治疗前后血清hs-CRP和胰岛素抵抗指数.结果 观察组手术时间、术中出血量和住院时间均少于对照组(P＜0.05).两组术后胰岛素抵抗指数与hs-CRP均有明显升高,但对照组比观察组升高更明显(P＜0.05).结论 腹腔镜胆囊切除术比小切口手术对hs-CRP和胰岛素抵抗影响更小.

목적 탐토복강경담낭절제술대담결석환자초민C-반응단백(hs-CRP)화이도소저항적영향.방법 장106례수술치료적담결석환자수궤분위관찰조화대조조,매조53례,관찰조채용복강경담낭절제술,대조조채용소절구담낭절제술,측정량조환자치료전후혈청hs-CRP화이도소저항지수.결과 관찰조수술시간、술중출혈량화주원시간균소우대조조(P＜0.05).량조술후이도소저항지수여hs-CRP균유명현승고,단대조조비관찰조승고경명현(P＜0.05).결론 복강경담낭절제술비소절구수술대hs-CRP화이도소저항영향경소.
Objective To study the effect of laparoscopic cholecystectomy on high-sensitivity Creactive protein(hs-CRP) level and insulin resistance in patients with gallstones.Methods One hundred and six patients with gallstones underwent operation were randomly divided into two groups,the observation group(53 cases) and the control group(53 cases).The patients in the observation group were operated through laparoscopic cholecystectomy,while the patients in the control group were operated through small incision cholecystectomy.The levels of serum hs-CRP and insulin resistance indexes were measured before and after treatment.Results Operative time,blood loss volume and length of stay in the observation group were less than those in the control group (P ＜ 0.05).The postoperative insulin resistance indexes and hs-CRP levels were significantly higher,but those in the control group increased more significantly (P ＜0.05).Conclnsions Laparoscopic cholecystectomy affect little on the hs-CRP level and insulin resistance than small incision.
