目的 探讨慢性阻塞性肺疾病评估测试(CAT)在慢性阻塞性肺疾病急性加重(AECOPD)住院期间的临床应用价值.方法 对AECOPD的住院患者在出院时进行肺功能检查,并应用CAT问卷进行症状评估,采用Pearson直线相关分析肺功能指标与CAT分值之间的相关性.根据肺功能对患者进行分级,并与根据CAT评分的分级进行比较,观察其对病情严重程度评估的一致性.结果 CAT总分与FEV1% pred呈显著负相关(r=-0.557,P<0.01).CAT评分随患者病情加重而逐渐增加,病情严重程度不同的3组COPD患者,其CAT总分分别为(18±6)分、(21±7)分和(25±6)分,组间比较的差异有统计学意义(H=39.368,P<0.01).根据CAT分级和根据肺功能COPD严重程度分级比较,两者具有较高一致性.结论 在AECOPD住院期间行肺功能检查及CAT评分,有助于病情严重程度的综合评估.

Objective To investigate the value of COPD assessment test for patients in hospital with an acute exacerbation of COPD (AECOPD).Methods Patients admitted to hospital with a clinical diagnosis of AECOPD had pulmonary function tests post-bronchodilator at discharge,and were assessed with COPD assessment test (CAT) on the same day.Pearson's correlation coefiqcients were calculated.The stages of COPD determined by CAT score and lung function were compared to observe their consistency in determining disease severity.Results CAT score increased with the severity of the disease.and was negatively correlated to FEV1 % pred (r =-0.557,P <0.01).CAT score varied significantly in patients (18±6,21±7 and 25±6) with different severity of COPD(H =39.368,P <0.01).There was a high degree of consistency between the stages of COPD determined by CAT score and lung function.Conclusions Pulmonary function tests and CAT assessment are useful for patients in hospital at the time of AECOPD to determinate severity of disease.
