目的 分析浙江省诸暨市人民医院2006-2010年期间鲍氏不动杆菌(Ab)的临床分布和耐药情况,为临床选择抗菌药物提供理论指导.方法 将来自该院2006年1月至2010年12月年门诊和住院患者的临床送检样本中培养分离出1961株Ab,采用K-B纸片扩散法对Ab进行药敏试验,对其耐药率变化进行比较,并分析其科室分布和样本来源情况.结果 1961株Ab的主要科室来源为ICU,占46.92% (920株);主要样本来源为痰,占90.52%(1775株).药敏试验显示Ab对头孢哌酮/舒巴坦耐药率最低,而氨霉素的耐药率最高.亚胺培南及美罗培南耐药率上升趋势最明显,分别从2006年的8.5%和6.0%增长到2010年的53.4%和65.0%.Ab对14种常见抗菌药物的耐药率在2006年至2010年期间呈逐年升高的趋势(P<0.05).结论 该院Ab的检出率逐年升高,其耐药情况有加重趋势,应加强对重点科室耐药监测及临床合理使用抗生素的指导.

Objective To surveillance clinical distribution and drug resistance of Acinetobacter baumannii (Ab) in Zhuji People's Hospital from 2006 to 2010 and provide evidences for treatment and prevention.Methods A total of 1961 clinical Ab strains were isolated from the outpatients' and inpatients' specimens in Zhuji People' s Hospital from January 2006 to December 2010.And all of the strains were cultured and identified.Drug susceptibility tests were performed by K-B disk diffusion method.Variation of resistance rate,clinical distribution and specimens source were analyzed.Results Most of the Ab isolates (46.92%) located in ICU.The main source of specimens was sputum (90.52%).The resistance rate of Ab to cefoperazone/sulbactam was the lowest,and to chloramphenicol was the highest.And the resistance rates of Ab to imipenem and meropenem increased from 8.5% and 6.0% in 2006 to 53.4%and 65.0% in 2010.The resistance rate of Ab to 14 commonly used antimicrobial agents had increased with years from 2006 to 2010 (P < 0.05).Conclusions The detection rate of Ab in Zhuji People's Hospital increases year by year,and drug resistance of Ab is becoming more and more serious,effective measures should be taken to imrove the antibiotics stewardship,rational use and the infection control.
