目的对比评价腹腔镜和局麻小切口精索内静脉高位结扎术手术治疗原发性精索静脉曲张的优缺点.方法回顾性分析523例生殖男科门诊行局麻小切口精索内静脉高位结扎术和63例同期于住院部行腹腔镜精索内静脉高位结扎术的病例.比较两组的手术时间、切口长度、抗生素使用时间、住院时间、术后并发症发生率等.结果与腹腔镜组相比较,小切口组住院时间及抗生素使用时间均较短,有统计学意义(P＜0.05)；两组在手术切口总长度、手术时间、术后复发率、术后并发症发生率方面的差异均无统计学意义(P＞0.05).结论局麻小切口精索内静脉高位结扎术治疗原发性精索静脉曲张经济、有效,可获得理想结果.

Objectives To compare the advantages and disadvantages of spermatic vein high ligation by laparoscopy with that by mini incisions under local anaesthesia in the treatment of primary varicocele.Methods Retrospective analysed the clinical data of 523 cases of varicocele treated by high ligation of the internal spermatic vein using mini incisions with local anaesthesia and another 63 cases that treated by laparoscope-guided high ligation of the internal spermatic vein,then compared the operation time,incision length,antibiotic use time,hospitalization time and postoperative complications between the two groups.Results Compared with the laparoscopic group,the hospitalization time and antibiotic use time were shorter in mini incisions group and the results showed significant differences.There were no significant differences in incision length,operation time,postoperative recurrence and postoperative complications between the two groups.Conclusions High ligation of the internal spermatic vein using mini incisions method,which can achieve desired results for the treatment of varicocele,is economical and effective.
