目的探讨前列腺增生伴结石术后顽固性症状的病因及治疗方法.方法回顾性分析2011年1月～2012年12月本院收治的前列腺增生(BPH)伴结石患者行经尿道前列腺电切(transurethral resection of the prostate,TURP)或绿激光汽化术后出现顽固性尿痛5例,伴尿频、尿急.门诊给予抗感染、止痛及降低膀胱过度活动药物治疗1个月后,尿频、尿急症状明显改善,而尿痛症状未改善,复查B超提示前列腺残留结石,再次行TURP术,清除残余结石.结果 5例患者全部顺利完成手术,拔除尿管后尿痛消失.无排尿困难、尿潴留、发热等并发症.复查B超示无残留结石.结论前列腺结石残留是前列腺增生术后顽固性尿痛症状重要原因,再次行TURP术是治疗的首选方法,安全、有效.
목적 탐토전렬선증생반결석술후완고성증상적병인급치료방법.방법 회고성분석2011년1월～2012년12월본원수치적전렬선증생(BPH)반결석환자행경뇨도전렬선전절(transurethral resection of the prostate,TURP)혹록격광기화술후출현완고성뇨통5례,반뇨빈、뇨급.문진급여항감염、지통급강저방광과도활동약물치료1개월후,뇨빈、뇨급증상명현개선,이뇨통증상미개선,복사B초제시전렬선잔류결석,재차행TURP술,청제잔여결석.결과 5례환자전부순리완성수술,발제뇨관후뇨통소실.무배뇨곤난、뇨저류、발열등병발증.복사B초시무잔류결석.결론 전렬선결석잔류시전렬선증생술후완고성뇨통증상중요원인,재차행TURP술시치료적수선방법,안전、유효.
Objectives To explore the cause and the treatment method of intractable symptoms after prostectomy for benign prostatic hyperplasia (BPH) with calculus.Methods The data of BPH with stones were retrospectively analyzed in our hospital.The 5 cases of them complined of symptoms of intractable urodynia after transurethral resection or photoselective vaporization of prostate,accompanied with frequent micturition,urgent micturition.Patients were treated with anti-infective,Analgesic therapy and reduce bladder excessive activity drug after 1 month.The symptoms of frequent micturition,urgent micturition have improved significantly.But urodynia has no obvious improvement.Reviewing type-B ultrasound showed residual stones.5 cases underwent reoperation and remove residual stones.Results All patients were successfully completed the operation.The urodynia disappears after catheter removal.No adverse reactions of Dysuria,urinary retention,fever have been found.Reviewing type-B ultrasound showed no residual stones.Conclusions The residual stones after prostatectomy is an important cause of intractable urodynia.Reoperative TURP is a safe and effective procedure in treatment of it.
