目的探讨前列腺特异性抗原(PSA)、前列腺特异性膜抗原(PSMA)、α-甲基酰基辅酶A消旋酶(P504S)在前列腺疾病组织中的表达差异及意义.方法选取惠州市中心人民医院病理科201 1年7月～2013年7月前列腺穿刺标本157例,其中前列腺腺癌(PCa)38例,前列腺上皮内瘤变(PIN) 45例,非典型腺瘤样增生(AAH) 11例,前列腺良性增生(BPH) 63例,应用免疫组织化学技术检测前列腺组织中PSA、PSMA、P504S的表达.结果 PSA在各种前列腺组织中均有表达;PSMA在前列腺腺癌、前列腺上皮内瘤变、前列腺非典型腺瘤样增生、前列腺良性增生组织中的阳性表达率分别为65.8％、42.2％、18.2％、9.5％;P504S在以上组织中的阳性表达率则分别为94.7％、13.3％、9.1％、0％.结论 P504S在前列腺腺癌组织中高表达,其阳性表达率高于PSMA,可为前列腺腺癌的诊断提供参考依据.

Objectives To investigate the expression and role of prostate specific antigen(PSA),prostate specific membrane antigen(PSMA) and alpha-methyl-CoA racemase (P504S) of prostatosis.Methods 157 samples of prostate were got from department of pathology of HuiZhou Municipal Central Hospital between July,2011 and July,2013,including 38 adenocarcinomatous of prostate(PCa),45 prostatic intraepithelial neoplasm(PIN),11 atypical adenoma hyperplasia (AAH) and 63 benign prostatic hyperplasia (BPH).Expression of PSA,PSMA and P504S were detected by immunohistochemical metheod.Results PSA expressed in all the prostate tissues.The positive expression rates of PSMA were 65.8％,42.2％,18.2％ and 9.5％ respectively in PCa,PIN,AAH and BPH.Accordingly,the expression rates of P504S were 94.7％,13.3％,9.1％ and 0％.Conclusions p504S has a higher specificity of expression in adenocarcinoma of prostate than PSMA,and may be an ideal tumor marker to raise the diagnosis rate of PC.
