近年的一些实验及临床观察表明,二甲双胍对糖尿病肾病有着独立于降糖作用之外的保护作用.相关机制可能包括抑制线粒体氧化呼吸链复合体Ⅰ,激活AMP活化蛋白激酶(AMPK),改善肾脏的氧化应激、炎性反应和高滤过状态,抑制晚期糖基化终末产物(AGEs)的生成,减轻肾脏脂质沉积以及肾小球硬化和间质纤维化等.对于合并肾脏并发症,估算的肾小球滤过率(eGFR)大于30 ml/(min·1.73m2)的糖尿病患者,可应用二甲双胍治疗.

Recently,a number of experimental and clinical observations suggest that metformin has a specific protective effect on diabetic nephropathy(DN) independently of its hypoglycemic effect.The mecha-nisms include activating AMP-activated protein kinase through a inhibition of the mitochondrial respiratory chain complex I,improving oxidative stress,inflammation and hyperfiltration of the kidney,reducing the for-mation of advanced glycation end products,lightening lipid deposition,glomerulosclerosis and renal interstitial fibrosis,etc.The use of metformin would be benefitial in DN patients with an estimated glomerular filtration rate higher than 30 ml/(min · 1.73m2).
