糖尿病视网膜病变(diabetic retinopathy,DR)的病理机制一直是研究的热点.经典理论认为视网膜微血管病变是DR最早的超微结构改变,但近年的研究表明,血-视网膜屏障的破坏和神经变性在DR的早期病变中也发挥着举足轻重的作用.甚至有学者发现在视网膜微血管病变以前就已出现了神经元的大量凋亡和胶质细胞的活化.

The pathological mechanisms of diabetic retinopathy (DR) have always been a research hot topic.It has been believed that retinal microvascular lesions are the earliest pathological change in DR.But recently,studies showed that blood-retinal barrier damage and nerve degeneration also play a decisive role in early DR pathological changes.Some authors found that a large number of apoptotic neurons and activated microglial cells appeared before the retinal microvascular lesions happened.Because early DR analysis is of great value for clinical diagnosis and treatment,this article reviews the research advances in early diabetic retinopathy physiopathology.
