目的 观察盐酸氨溴索对百草枯中毒致急性肺损伤大鼠肺表面活性物质蛋白-D(alveolar surface active substance-D,SP-D)及各相关指标的影响,并评价盐酸氨溴索对百草枯中毒治疗作用.方法 将120只雄性大鼠,随机(随机数字法)分为3组:生理盐水组(NS组),百草枯中毒组(PQ组),盐酸氨溴索治疗组(AT组);再分别将每组按时间点(第1、3、5、7天)随机(随机数字法)分为4组.各组以Western blot法检测肺组织SP-D蛋白含量,以HE染色观察大鼠肺组织形态学变化,并同时检测各组大鼠的白细胞计数(white blood count,WBC)和中性粒细胞(polymorphonuclear,PMN)百分比及动脉血氧分压(arterial oxygen pressure,Pa02).结果 AT组第3、5、7天WBC计数均低于PQ组,高于NS组各相应时间点,差异有统计学意义(P<0.01);AT组与PQ组相比PMN百分比,第3、5、7天下降,但高于NS组各相应时间点,差异有统计学意义(P<0.01);AT组Pa02各相应时间点均低于NS组(P<0.01),第3、5、7天AT组Pa02高于PQ组,差异有统计学意义(P<0.01);HE染色结果显示三组大鼠肺组织形态学变化具有明显不同;AT组肺组织SP-D含量第5、7天高于PQ组,而低于NS组各相应时间点,差异具·有统计学意义(P<0.01).结论 盐酸氨溴索具有抗炎作用及促进肺泡Ⅱ型细胞表面相关活性物质SP-D蛋白合成与分泌,对百草枯中毒所致急性肺损伤有一定治疗效果.

Objective To observe the effects of ambroxol hydrochloride (AH) on suffactant proteinD (SP-D) changes in rats with acute lung injury (ALI) from paraquat (PQ) poisoning in order to evaluate the therapeutic effect of AH.Methods One hundred and twenty male rats were randomly (random number) divided into three groups,namely NS (normal saline) group,PQ group,and AT (ambroxol treat) group.On the 1st,3rd,5th and 7th day after PQ poisoning,blood samples and lung tissue were taken for laboratory examination.Venous blood samples were taken to detect WBC count and percentage of PMN ; and arterial blood for measuring PaO2.The rat lung tissue morphological changes were observed with HE staining,and lung tissue surfactant protein-D (SP-D) content was determined with Western blot.Results The numbers of WBCs in AT group were lower than those in PQ group and higher than those in NS group on the 3rd,5th and 7th day (P <0.01).Compared with PQ group,the percentages of PMN in AT group were decreased obviously on the 3rd,5th and 7th day (P <0.01),but significant higher than those in NS group (P <0.01).The levels of PaO2 in AT group were lower than those in NS group at the same intervals (P <0.01).Compared with the PQ group,the levels of PaO2 in AT group were higher on the 3rd,5th and 7th day (P < 0.01).The magnitude of histopathological changes of lung tissue among three groups were obviously different; the levels of SP-D in the lung tissue in AT group were higher than those in PQ group,but lower than those in NS group at the same intervals (P < 0.01).Conclusions The ambroxol hydrochloride had anti-inflammatory effect owing to pulmonary aleolar epithelial type Ⅱ cells secreting synthetic SP-D protecting the rats from paraquat poisoning.
