目的评估多学科护理照顾模式对老年卒中门诊患者干预的有效性.方法该模式为老年卒中门诊患者提供半天的门诊随访,包括跨学科评估及护理.该项目评价指标包括护理照顾效果、患者满意度及护理照顾者满意度.结果患者在初次和末次门诊随访收缩压发生显著变化[(139.2±19.7)mm Hg比(127.2±17.8)mm Hg],舒张压发生显著变化[(78.2±12.3) mm Hg比(72.3±10.4)mm Hg],低密度脂蛋白也呈下降趋势[(102.9±20.4) mg/dl比(87.6±18.0) mg/dl].FIM评分改善[(103.11±18.67)分比(117.37 ±17.47)分].在推荐的护理内容中,每个患者至少有一项内容发生较大的变化.卒中患者和其护理照顾者对多学科卒中护理模式满意.结论对老年卒中患者提供多学科护理照顾模式能够明显提高护理照顾的质量.

Objective To evaluate the effectiveness of multidisciplinary care pattern on elderly stroke outpatients.Methods Multidisciplinary care pattern provided elderly stroke outpatients with half-day clinic follow-up,including interdisciplinary evaluation and care.The evaluation index included the effect of nursing,satisfaction of patients as well as satisfaction of caretakers.Results Both systolic and diastolic blood pressure had been improved in the last follow-up compared with the first [(127.2 ± 17.8) mm Hg vs (139.2 ± 19.7)mm Hg and (72.3 ± 10.4)mm Hg vs (78.2 ± 12.3)mm Hg].Low density lipoprotein reduced from (102.9 ±20.4) mg/dl to (87.6 ± 18.0) mg/dl,and FIM score also improved from (103.11 ± 18.67) to (117.37 ±17.47).Among recommended nursing contents,at least one subject had significant change for one patient.Both patients and caretakers were satisfied with the care pattern.Conclusions Multidisciplinary care pattern can obviously improve the quality of nursing for elderly stroke outpatients.
