目的 探讨集束化干预在超高龄经皮内镜下胃造瘘术(PEG)患者中的应用效果.方法 将解放军总医院消化科2011年1-12月采用常规护理的24例超高龄PEG患者作为对照组,2012年1-12月采用集束化干预的25例超高龄PEG患者作为干预组,对两组患者住院期间护理效果进行评估.结果 干预组脱管、皮肤感染、消化道不良反应发生率(0.00％,4.00％,16％)明显低于对照组(8.33％,12.50％,25％),两组比较差异有统计学意义(x2值分别为2.27,5.17,8.92;P ＜0.05).结论 集束化干预应用于超高龄PEG患者,可降低患者脱管、皮肤感染、消化道不良反应发生率,提高护理质量.

Objective To study the effect of cluster intervention on reducing the common complications in super-aged patients with percutaneous endoscopic gastrostomy (PEG).Methods Totals of 24 PEG patients as control group that received regular care between January and December of 2011,and 25 cases received cluster intervention as intervention group between January and December of 2012,the effect of nursing during hospitalization in patients were evaluated and analyzed.Results In intervention group,after the intervention,the incidence of taking off the tube (0.00％ vs 8.33％),skin infections (4.00％ vs 12.5％),and gastrointestinal adverse reactions (16％ vs 25％) were significantly lower than that of the control group (x2 =2.27,5.17,8.92,respectively;P ＜ 0.05).Conclusions The cluster nursing intervention for aged patients with PEG can reduce the incidence of taking off the tube,skin infections,and gastrointestinal adverse reactions,and improve the quality of nursing.
