目的 比较有创血压与无创血压监测在新生儿外周动静脉同步换血中的临床效果.方法 采用方便抽样方法,对11例新生儿高胆红素患儿换血治疗中进行109组有创和无创两种测压方法的监测,比较两种方法在换血治疗中的临床效果,并对相关因素进行分析.结果 11例患儿换血治疗中无创血压平均值为(50.38±1.76) mm Hg,有创血压平均值为(48.06 ±2.18)mm Hg,无创血压平均值略高于有创血压平均值,两种方法所测量数值之间呈正相关(r=0.82,P<0.05);出生体重越小,两种测量结果的差异越大(t=-2.53,P<0.05).结论 有创血压监测能更准确提供患儿血压的变化值,为疾病预测提供可靠依据,同时还可避免反复穿动脉穿刺,具有良好的临床监测效果和可行性.

Objective To compare the effect of invasive and non-invasive blood pressure monitoring on exchange transfusion of neonate.Methods A convenience sampling was used for data collection.11cases with exchange transfusion via peripheral vessels for hyperbilirubinemia were recruited and 109 times of invasive and non-invasive blood pressure monitoring were recorded.Then,the data was analyzed.Results The main findings from this study were:the data from non-invasive method was higher than invasive[(50.38 ± 1.76) vs (48.06 ±2.18)mm Hg] ; two methods of blood pressure monitoring had positive relationship(r =0.82,P <0.05) ;birth weight of baby was negatively associated with difference between two methods (t =-2.53,P < 0.05).Conclusions Invasive blood pressure monitoring is the first choice for neonatal with exchange transfusion.
