目的 探讨聚焦解决模式在胰岛素笔自我注射的2型糖尿病患者中的应用效果.方法 选择2011年4-6月符合纳入标准的2型糖尿病患者124例.对患者实施常规指导后按照随机数字表法随机分为研究组和对照组各62例,对照组采取常规教育,研究组采用聚焦解决模式干预,采用Mann-Whitney U秩和检验和x2检验、似然比x2检验比较干预前后差异.结果 干预后,研究组患者胰岛素注射知识和技能的掌握得分为29分,对照组为22分,差异有统计学意义(Z=-9.835,P<0.05);干预后研究组及对照组胰岛素注射知识和技能的掌握方面得分均高于干预前,差异有统计学意义(Z=-6.865,-6.014;P <0.05).干预后研究组注射疼痛、药液渗漏、局部肿胀、皮下硬结、皮肤瘙痒、局部淤血不良反应发生率低于对照组,差异有统计学意义(x2值分别为17.285,14.946,6.624,9.688,8.623,12.738;P <0.05).结论 应用聚焦解决模式教育干预能有效提高2型糖尿病患者对胰岛素注射知识和技能的掌握,减少患者注射局部不良反应的发生.

Objective To discuss the effect of solution focused approach on patients with type 2 diabetes who use insulin pen for self-injection.Methods Totals of 124 patients with type 2 diabetes in the outpatient clinic of Affiliated Hospital of Nantong University from April to June 2011 were selected and randomly divided into the experimental group and the control group after regular guidance.The control group received conventional education,while the experimental group was intervened with solution focused approach.MannWhitney U test and x2 test were used to compare the differences before and after intervention.Results After intervention,patients' mastery of knowledge and skills of insulin injection was 29 in the experimental group and 22 in the control group,with statistically significant difference (Z =-9.835,P < 0.05).And the mastery in both the experimental and control group was better after intervention than before,and the differences were statistically significant (Z =-6.865,-6.014,respectively ;P < 0.05).After intervention,the incidence rate of adverse reactions such as injection pain,liquid leakage,local swelling,subcutaneous induration,pruritus and local congestion was obviously lower in the experimental group than in the control group,and the differences were statistically significant (x2 =17.285,14.946,6.624,9.688,8.623,12.738,respectively; P < 0.05).Conclusions Solution focused approach can improve the mastery of knowledge and skills of insulin injection of patients with type 2 diabetes and reduce their incidence of adverse reactions of local injection.
