目的 探讨柔性管理在社区卫生服务护理管理中的应用效果.方法 以管理中心下辖39个社区卫生服务中心护理人员101人和家庭病床120患者例作为研究对象,管理中心按随机数字法进行排序登记建档,排列序号奇数的社区卫生服务中心护理人员和家庭病床患者为实验组,排列序号偶数作为对照组.实验组实施柔性管理,对照组实施常规护理.比较柔性管理对护士归属感、服务质量及护理满意度变化.结果 实施柔性管理后,实验组与对照组比较,护士归属感荣誉率分别为37.2%,2.0%;工作和学习热情显著提高,自考率分别为45%,18.0%,离职率分别为0.0%,14.0%,差异有统计学意义(x2值分别2.67,2.31,2.16;P<0.05);实验组护理质量评分分别为基础护理(96.0±0.7)分、病床管理(96.0±0.3)分、护理文件(97.1±0.1)分、护理操作(92.5±0.1)分,实验组较对照组明显提高,差异均有统计学意义(t值分别0.87,0.92,0.96,2.13,P<0.05);实验组患者的满意度91.7%,显著高于对照组的68.3%,差异有统计学意义(x2 =5.66,P<0.05).结论 柔性管理在社区健康服务中心护理管理中的应用效果显著,能提高护士的自考虑、荣誉获得率及护理质量,降低其离职率,提高患者对护理工作的满意度,值得临床推广应用.

Objective To investigate the application and effect of flexible management in nursing manage.Methods The nursing staffs of 101 and 120 patients using family beds from 39 community health service centers under the jurisdiction of Management Centre were selected as the research object.All of them were sorted and registered by using random number method,and were divided into two groups.Those with odd number were put in the experimental group with flexible management,and those with even number were put in the control group with routine nursing.The changes of nurses' belonging sense,service quality and patients satisfaction with nursing under application of flexible management were observed and evaluated.Results After the implementation of flexibility management,the honor rate of nurses' belonging sense,the rate of taking voluntarily examinations and the turnover rate which reflected enthusiasm for working and learning,were 37.2%,45% and 0.0% in the experimental group,and 2.0%,18.0% and 14.0 % in the control group,respectively,and these differences were statistically significant between the two groups (x2 =2.67,2.31,2.16,respectively; P < 0.05).The score of basic nursing,the score of hospital bed management,the score of nursing documentation and the score of nursing operation in the experimental group were (96.0 ± 0.7),(96.0 ± 0.3),(97.1 ± 0.1) and (92.5 ± 0.1),respectively,which were higher those in the control group,and these differences were statistically significant between the two groups (t =0.87,0.92,0.96,2.13,respectively ; P < 0.05).The patients satisfaction of 91.7% in the experimental group was higher 68.3% in the control group,the difference was statistically significant (x2 =5.66,P < 0.05).Conclusions The application of flexible management in community nursing management has achieved the significant effect,which can help to enhance the rate of taking voluntarily examinations,the honor rate of nurses' belonging sense and the nursing quality,reduce the turnover rate of nurses,and improve patients satisfaction with nursing.It is suitable for more application and promotion in clinical practice.
