目的 探讨超声造影在检测肝细胞癌血流灌注特征中的应用价值.方法 78例肝细胞癌患者行彩色多普勒血流显像(CDFI)和能量多普勒(PDI)检查,比较注射造影剂前后肿瘤血管显像情况,分析血流平均密度(FMD),定量比较血流信号增强效应.结果 造影后CDFI和PDI显示肝细胞癌病灶内部及周边血流信号明显增强,血管形态较造影前清晰完整.造影前PDI FMD为0.104±0.081,造影后为0.234±0.172,差异有统计学意义(P<0.0l).造影前CDFI FMD为0.084±0.099,造影后为0.172±0.114,差异有统计学意义(P<0.01).造影前后PDI和CDFI FMD比较差异均有统计学意义(P<0.05).结论 超声造影的应用有助于更准确地评价肝细胞癌的血供情况,超声造影后PDI血流显像优于CDFI,对肝细胞癌的诊断有重要价值.

Objective To explore the application of contrast-enhanced ultrasound (CEUS) on the vascular character of hepatocellular carcinoma blood flow perfusion.Methods Seventy-eight patients with hepatocellular carcinoma were performed by CEUS.After intravenous administration of contrast agent,color Doppler imaging (CDFI) and power Doppler imaging (PDI) were used to evaluate the blood flow at the periphery and the inside of the lesion.Flow mean density (FMD) was analyzed and quantitative compared with Doppler enhanced intensity.Results After injection,the number of flow messages increased both at the periphery and the inside of hepatocellular carcinoma lesion under the mode of CDFI and PDI,the whole blood distribution became clearer.After injection,the PDI FMD was increased significantly (0.234 ± 0.172 vs.0.104 ±0.081,P<0.01),and CDFI FMD was increased significantly also (0.172 ±0.114 vs.0.084 ±0.099,P<0.01).There was significant difference between CDFI and PDI before and after injection (P<0.05).Conclusions CEUS can contribute to accurately evaluate the blood supply of hepatocellular carcinoma and PDI can show blood flow signal more clearer than CDFI.CEUS has important value in the diagnosis of hepatocellular carcinoma.
