目的 观察角膜板层切除联合羊膜移植及结膜瓣覆盖术治疗真菌性角膜炎的临床效果.方法 病灶区角膜板层切除,保存或新鲜羊膜氟康唑注射液内浸泡半小时后双层或多层覆盖角膜植床,取上方桥状或蒂状球结膜瓣覆盖病灶区连同羊膜一并缝合固定,1个月后在表面麻醉下剪断结膜蒂,并拆线.结果 19例(19眼)角膜炎症全部控制,拆线时羊膜吸收或部分吸收,角结膜愈合.6个月~3年随访,视力数指~0.1者8眼,0.2~0.5者11眼.所有移植的结膜瓣均为半透明愈合,血管萎缩,大部分角膜表面凹陷明显变浅.结论 本手术方法为一种治疗真菌性角膜炎的有效方法之一.
目的 觀察角膜闆層切除聯閤羊膜移植及結膜瓣覆蓋術治療真菌性角膜炎的臨床效果.方法 病竈區角膜闆層切除,保存或新鮮羊膜氟康唑註射液內浸泡半小時後雙層或多層覆蓋角膜植床,取上方橋狀或蒂狀毬結膜瓣覆蓋病竈區連同羊膜一併縫閤固定,1箇月後在錶麵痳醉下剪斷結膜蒂,併拆線.結果 19例(19眼)角膜炎癥全部控製,拆線時羊膜吸收或部分吸收,角結膜愈閤.6箇月~3年隨訪,視力數指~0.1者8眼,0.2~0.5者11眼.所有移植的結膜瓣均為半透明愈閤,血管萎縮,大部分角膜錶麵凹陷明顯變淺.結論 本手術方法為一種治療真菌性角膜炎的有效方法之一.
목적 관찰각막판층절제연합양막이식급결막판복개술치료진균성각막염적림상효과.방법 병조구각막판층절제,보존혹신선양막불강서주사액내침포반소시후쌍층혹다층복개각막식상,취상방교상혹체상구결막판복개병조구련동양막일병봉합고정,1개월후재표면마취하전단결막체,병탁선.결과 19례(19안)각막염증전부공제,탁선시양막흡수혹부분흡수,각결막유합.6개월~3년수방,시력수지~0.1자8안,0.2~0.5자11안.소유이식적결막판균위반투명유합,혈관위축,대부분각막표면요함명현변천.결론 본수술방법위일충치료진균성각막염적유효방법지일.
Objective To evaluate the effect of corneal lamellar resection plus amiotic membrane (AM) transplantation combind with conjunctival flap covering for fungal keratitis. Methods The corneal lamella of focus was resected and covered with preserved or fresh amniotic membrane which was immersed in Fluconazole injection solution for half an hour and conjunctival flap. The AM and conjunctival flap were fixed up by suture. The conjunctival pedicle was snipped under surface anaesthesia and removed the suture after one motth. Results Keratitis of 19 eyes was controled, AM was totally or partly absorbed. Conjunctiva and cornea healed up when suture was removed. Following-up 6 months to 3 years, visual acuities of 8 eyes were CF ~ 0.1,11 eyes were 0.2 ~ 0.5. All of conjunctival flaps were translucent in healing up. Blood vessels were atrophic, and most hollows of corneal surface became shallow. Conclusions This measure is one of efficient ways for treatment of fungal keratitis.
