目的 评价多孔聚乙烯(Medpor)下睑插片作为下睑植入物治疗甲状腺相关性眼病引起的下睑退缩倒睫的效果.方法 甲状腺相关性眼病8例(12眼),甲亢控制6个月以上.术前测量在第一眼位下睑相对角膜下缘的距离及距下眶缘的距离.手术采用下睑袋皮肤切口,打开眶隔,切断下睑缩肌止端,视患者下睑退缩程度修剪Medpor插片的形状及大小,将植入物置于下睑板下缘及眶下缘骨膜之间.术后观察下睑位置、倒睫矫正情况、角膜情况及植入物是否移位或脱出.结果 对8例12眼行Medpor下睑插片植入者随访12个月,下睑退缩及倒睫矫正良好,角膜病变愈合或好转,无植入物移位.12眼在第一眼位下睑缘位于角膜下缘或角膜下缘上下0.5 mm,1眼植入物暴露,经再次手术后,效果良好.结论 Medpor下睑插片作为植入物,组织相容性好,硬度足够,可提供下睑长期支撑,达到良好的功能及美容效果.Medpor下睑插片可作为治疗甲状腺相关眼病下睑退缩倒睫的较理想植入物.

Objective To evaluate the efficacy of Medpor as a spacer graft in the treatment of the lower eyelid retraction and trichiasis caused by thyroid-associated-ophthalmopathy.Methods A retrospective case series on 12 eyes of 8 patients with thyroid-associated-ophthalmopathy was analyzed.All patients' hyperthyroidisms have been under control for at least 6 months.Preoperative measurements of the relative distances between lower eyelid and the inferior cornea limbus,and between lower eyelid and the inferior orbital rim at the primary position of eye were performed.During the operation,skin incision was performed in lower eyelid bag,and then lower eyelid retractor side was cut off after the orbital septum was opened up.Medpor spacer graft was then trimmed to the exact size and shape based on patient' s degree of lower eyelid retraction,and the spacer graft was implanted between the lower edge of inferior tarsus and inferior orbital rim periosteum.The position of lower eyelid,the condition of trichiasis rectification,the condition of cornea,and whether the spacer graft has displaced or prolapsed were observed post-operation.Results 8 patients with 12 eyes that were implanted with Medpor were followed up for 12 months.It was found that the lower eyelid retraction and trichiasis were properly rectified,and keratopathy was cured or improved.No implant displacement was found.When all 12 eyes were at the primary position of the eye,the lower lid margin is located at or within the range of 0.5mm of the inferior corneal limbus,The spacer graft in one eye was exposed post-operation,but positive outcome was seen after reoperation.Conclusion Medpor,as a spacer graft,has good histocompatibility with surrounding tissues,has suitable hardness,and can provide long-term support to the lower eyelid,and achieve good functional and cosmetic effects.Medpor can be used as an ideal spacer graft in the treatment of the lower eyelid retraction and trichiasis in thyroid-associated-ophthalmopathy.
