目的 了解老年性白内障患者围手术期使用抗生素滴眼液前后口咽部细菌分布与耐药性的变化情况.方法 前瞻性观察研究老年性白内障手术110例(110眼),分别于用药前、用药(10±1)d与停药(23 ±3)d 3个时间点取咽拭子送检,对分离菌株做鉴定与药敏试验,结果作统计学分析.结果 手术前后共收集318份咽拭子标本,细菌培养阳性率为100%,共检测出15种细菌;检出率最高的4种菌为甲型链球菌、卡他奈瑟菌、表皮葡萄球菌与肺炎克雷伯菌;在用药(10±1)d与停药(23±3)d时,有部分菌群的检出率出现下降与回升的现象;甲型链球菌、表皮葡萄球菌对环丙沙星的耐药率在用药(10±1)d时出现上升.结论 抗生素滴眼液对口咽部细菌检出率存在一定影响并有可能改变细菌的耐药率.

Objective To investigate the distribution and drug resistance of oropharyngeal flora in patients with senile cataract undergoing antibiotic eyedrops used for perioperative period.Methods Prospective and observational study design.Totally 110 cases(110 eyes) with senile cataract were enrolled in this study.An oropharyngeal swabs were taken from each patient at three time point:before medications,after application of antibiotic eye drops (10 ± 1) d and after stopping medication (23 ± 3) d.The bacterial culture,identification,distribution and drug resistance were statistical analyzed.Results Totally 318 throat swab samples were collected and 15 kinds of bacteria were isolated.The prevalence of oropharyngeal bacterial carriage was 100%.The top four bacteria were alpha hemolytic streptococcus,Moraxella catarrhalis,staphylococcus epidermidis and Klebsiella pneumoniae.There were some positive rates of bacteria showed an downward tendency at the second time-point and some showed an upward tendency at endpoint.The rates of resistance to Ciprofloxacin increased by alpha hemolytic streptococcus and staphylococcus epidermidis after application of antibiotic eye drops (10 ± 1) d.Conclusion The antibiotic eye drops may affect the detected rates and drug resistance of oropharyngeal bacteria.
